A major theme of the series is compassion. Like Tavore, Gesler now has compassion for Kaminsod, who has caused much of the suffering throughout the series, but has been literally ripped apart for thousands of years. He was torn apart when he arrived in the Malazan world from his own world; it is wrong for him to remain among the Malazans. Though Kaminsod may be destroyed when he returns to his world, he would rather return than remain how he is. The Crippled God wants to write a "Book of the Fallen" to commemorate those who died to defend him. Back in "Dust of Dreams", Badalle wanted to do this. So, the series title "Book of the Fallen" refers not only to the dead, but the person who wants to write (and possibly writes?) it, the Fallen God, Kaminsod.

2. What is the Crippled God's real name?

Kaminsod is not of the Malazan world and was torn apart many thousands of years ago when he arrived. As a side effect of reading the Deck in a land that uses another magic system, Tiles, disturbances take place all over, including transforming Curdle and Telorast into full dragons. Oh, and Ublala punches the god Errant in the face to save Brys' life.

The book is third-person omniscient, following the actions and thoughts of dozens of characters. However, Kruppe often tells the story in his own style, in a lighter, somewhat amused tone distinct from other sections; chapters often begin and end with commentary from him. Kruppe never uses first-person pronouns when he talks (a trait I personally find annoying), so the sections aren't in first person. None of the previous "Malazan" books employed this technique.

Hellian is a young sergeant. In one fight where she's being shot at and loses her shield (actually the top of a barrel), she goes for a wine bottle so she can have one last swig before she dies. It may well be the latter--an unknown woman appears to her after this engagement, then vanishes in an instant.

Blood-oil may be related to otataral dust, as both negate magic.

Because of rampant cannibalism, peasant starvation, and a few other cruel practices, Rake thinks that the Pannion Domin is ultimately unsustainable. I'm inclined to agree with him.

Tattersail dies partway through "Gardens of the Moon". Oakhallow is a character from Elizabeth Moon's "The Deed of Paksenarrion" trilogy, and Fireheart is from Erin Hunter's "Warriors" series.

That Assail was Calm, and she has returned to help her fellow Assail.

Otataral ore prevents and negates magic. The otataral doesn't kill her, though--it merely prevents her from escaping into a warren when Degaroth come and tear her apart. Interestingly, in her dying thoughts, Poliel, Mistress of Pestilence, reveals that she wanted to heal Burn, and spreading disease that killed humans was her way of doing that.
