Sean was obviously touched by the story and tried to console Ellie, saying "I am so sorry." Ellie replied by asking, "Doctor, have you ever hurt anyone you love? In such a way that doesn't even seem like you, but then you can't go back and make it different. It's a kind of pain I never could have imagined." Though Sean clearly felt sympathetic and related Ellie's story to his own problems with Sean, he told her he couldn't risk the surgery considering the danger her age posed.

After confronting Sean and Julia about their questionable parenting, Christian entered the operating room to find Quentin scrubbed in and ready to assist. When Quentin confirmed that he had been, it was clear that Matt felt somewhat conflicted, so Christian left the operating room on his own.

When they began the consultation, Sean and Christian didn't know that Dr. Forsythe was there about Kiki, and when she made it clear she was seeking surgery for the gorilla, the doctors quickly blew her off. Dr. Forsythe insisted that the surgery was vital because Kiki needed to look normal when she met her chosen mate. Sean questioned that the gorilla could have a "self-image problem," but Dr. Forsythe pointed out that apes were the only other species that could recognize themselves in a mirror and explained that Kiki could see that something was different. She told the doctors that she'd been working with Kiki since she was a baby and had been rescued after poachers had killed her mother (the same attack was the source of Kiki's scar). Dr. Forsythe's work had focused on inter-species communication and had been quite successful, but Kiki needed to have a baby so they could discover whether or not Kiki would pass on her acquired language skills to her offspring. The surgery was imperative because Kiki's mate would see the scar, recognize a disadvantage in Kiki, and attack or even kill her.

He identified the body, then the dream skipped to his funeral, where Julia, Kimber, Liz, and Sean all gave speeches. This seemed enough to convince Sean to do the surgery, since the combination of Ellie regret and Carl's loss played perfectly into Sean's pain concerning his own family.

Matt was taken to the police station (presumably after he did the right thing by calling them) and his parents were called so he could be questioned as a suspect. After Sean told Matt about Ava's vaginal reconstruction and 16-year-old gender reassignment surgery, Matt was furious and stormed out. The detective realized that the family had more on their mind than Adrian and let them go.

After Sean and Christian told her she may lose her legs if the infection was incurable, she seemed ready to break down, asking, "How am I supposed to get off this couch if I can't even walk?" Christian took off his gloves and mask and offered his own hand to Momma, which seemed to renew her strength a bit.

Then Christian watched as Sean's expression turned from hopeful openness to shocked dismay before putting his shades back on and walking off alone.
