Quiz Answer Key and Fun Facts
1. The underlying cause of Chiari symptoms is a herniation of the cerebral tonsils.

Answer: True

It can be caused by a congenital defect or be acquired later in life but the cause is still the same. The brain stem is compressed.
2. All symptomatic patients look sick.

Answer: false

It's an invisible disability because patients with it don't look like they have anything wrong. There are patients that are obviously disabled by this condition too, but it doesn't happen to all patients.
3. How many types of Chiari malformations are there?

Answer: 4 (5 if you include type 0)

Type 1 is the most common. Type 2 is also known as Arnold Chiari malformation. Some sources erroneously use "Arnold Chiari malformation" for all Chiari malformations.
4. All Chiari patients need surgery.

Answer: False

Some Chiari patients actually are asymptomatic (have no symptoms) and just need monitoring and possibly pain management for headaches. The problem is many doctors assume every Chiari patient is asymptomatic till proven otherwise, which is why most patients don't get told their diagnosis on the first visit.
5. Is there a cure for Chiari?

Answer: No

Surgery and physicial therapy relieve symptoms and slow down problems, but there is no cure.
6. Which disorder is NOT commonly misdiagnosed to a patient with Chiari One Malformation?

Answer: Cold

It is common for patients with Chiari to be misdiagnosed with everything but the cold as the cause of all their symptoms. Some reasons for the misdiagnosed Chronic Fatigue Syndrome is because patients are actually easily tired.
7. Most patients get a correct diagnosis of Chiari on the first doctor's visit.

Answer: False

Patients are lucky if they ever get a correct diagnosis post MRI without reading their MRI reports. The lesson: read your MRI reports even if you don't think you have Chiari malformation.
8. Patients with 5 mm Chiari are usually asymptomatic and patients with 20 or more mm are normally severly symptomatic.

Answer: False

This is one of the reasons many doctors don't tell their patients they have Chiari. The patients with the small herniations are more likely to be symptomatic than the ones with large herniations.
9. What surgery is commonly done for Chiari patients first when necessary?

Answer: Decompression

Shunt surgery is usually done when decompression surgery fails. Since the cause of Chiari is the compression of the brain stem, the goal of the surgery is to decompress the brain stem.
10. Which disorder is NOT common with Chiari Malformations?

Answer: Kienbock's Disease

Kienbock's is a rare bone disorder of the wrists. Some symptoms are similar, so it might actually be related and patients don't know it, and neither do doctors. Tethered cord syndrome is an affliction of the lower spine and it causes incontinence and strength loss in extremities. Elhers-Danlos syndrome (EDS) patients are usually double jointed and have skin that can be stretched.

Not everyone who is double jointed has EDS.
