The obvious answer, I think, is the fictional Long John Silver in R. L. Stevenson's "Treasure Island" though he wasn't a captain:

"His left leg was cut off close by the hip, and under the left shoulder he carried a crutch, which he managed with wonderful dexterity, hopping about upon it like a bird. He was very tall and strong, with a face as big as a ham - plain and pale, but intelligent and smiling."

Hector Barbossa, the fictional pirate captain of the Black Pearl in the "Pirates of the Caribbean" movies, had a wooden leg.

Captain Ahab, the captain in Melville's "Moby Dick", had false leg but it was made of whale bone.
