One armed shoulder holsters are designed to stay in place by wearing a garment, such as, a jacket over it.

On the other hand Jack Lord was pretty tough, so it could have been nailed on and he probably wouldn't have noticed or cared!

In this picture, where Jack Lord is without a jacket the holster is under tension with the lower end appearing to touch, or to be attached to the top of his trousers or perhaps belt.
