Answer: Palpatine

In the beginning of the novel, Anakin and Obi-Wan are on their way to try and rescue Chancellor Palpatine from General Grievous. Grievous had kidnapped the Chancellor shortly before, and was preparing to get him out of Republic space. This is the same in the movie, as well.

Answer: Kit Fisto's head

Anakin rushes in and finds the Nautolan Jedi Master's head with his trademark grin on it on the Chancellor's desk. In the movie, Fisto is slashed across the chest and falls in front of the door. Anakin thought that Fisto must have found death funny as well when he saw the Jedi Master's smile.

Answer: Lightsaber through the forehead

In the movie Kolar meets his end with a slash across the chest; in the book Palpatine stabs him through the forehead. After he is stabbed, he says, "It doesn't hurt much." He then crumples to the floor, his lightsaber deactivating as it hits the ground.

Answer: Cin Drallig

Obi-Wan remarks, "Look, Master... it's the Troll." Drallig had been killed by Anakin Skywalker as he protected the younger students of the order. He was cleaved through the shoulder by a lightsaber, and fell. The last thing he saw was his students being shot down by the clones which had turned against them.

Answer: By disguising themselves and killing Decoy Squad Five

Kenobi disguises himself as a hermit, and Yoda is disguised as a Jedi "baby" in his arms. When the clones come to investigate, Yoda activates his lightsaber and the duo give the clones a chance to surrender. The clones do not, and the two Jedi dispatch them, thus gaining access to the Temple.

Answer: Anakin

Just as in the movie, Anakin kills Count Dooku in the novel. This marks yet another footstep on Anakin's journey towards the dark side. Just as in the movie, Anakin was goaded by Palpatine into killing Dooku.

Answer: "Unless one of your happens to be carrying Yoda in his pocket, I hardly think I shall need one."

Dooku was extremely confident in his ability to dispatch both Jedi, despite his advancing age. He proudly proclaimed that unless either of them had brought Yoda, he would have no reason to surrender, as he would surely kill them. Dooku's arrogance proved to be his undoing.

Answer: That treachery is the way of the Sith

Dooku realizes that everything Sidious has been feeding him has been a lie. Only too late does Dooku understand that he was never going to live to see the new order that Sidious had told him about. Anakin has been set to be Dooku's replacement for years. Finally, Sidious has gotten Anakin to do his dirty work for him.

Answer: Yoda

Windu wishes to get the blessing of the Grand Master of the order before acting on Skywalker's news of Palpatine being the Sith lord. Yoda gives Windu his permission, and tells Windu, "May the Force be with you." Windu then sets out to arrest the Dark Lord of the Sith and restore peace to the Republic and the Jedi Order. He fails.

Answer: Darth Vader

Sidious knights Anakin as Darth Vader, Dark Lord of the Sith. The newly appointed Lord Vader then heads to the Jedi Temple and slaughters almost every single Jedi there, including the younglings who never stood a chance against Vader's blade. After the massacre in the temple, Vader sets out to slaughter the Separatist Council, whom Sidious had sent to Mustafar ... so that they would be 'safe'.
