Part of Lilith's trademark is her sharp, clear cut appearance which normally includes professional attire and hair pulled into a tight bun. When Lilith wears her hair down, Frasier finds her incredibly attractive which she often uses to her advantage.

After Diane dumps Frasier, he asks Lilith, a fellow colleague, out on a date and he takes her to Cheers. However, she finds him egotistical and the two bicker the entire time. She leaves in the middle of the date, which upsets Frasier. She re-appears in the season five episode "Abnormal Psychology" where their apparent hatred is revealed to be mutual attraction.

After the birth of Frederick, Lilith and Frasier agree to raise him Jewish as Frasier has no ties to any other particular religion. In "For Real Men Only" they host a Bris for Frederick at their house but when Frasier temporarily gets cold feet, the Bris ceremony winds up taking place in the Cheers bar.

After getting into a fight with Frasier over a pre-nuptial agreement, Lilith wins Sam in the charity auction as a way of "getting back" at Frasier. Sam, who doesn't want to rock the boat, agrees to let Frasier burst in on their weekend getaway to win Lilith back. But Lilith had no intention of the weekend going anywhere, and is able to convince Frasier to forget about the pre-nup.

Lilith goes into labor at Cheers, and Sam and Frasier rush her to the hospital. After the doctor tells her that it was a false alarm, Sam and Frasier stay to provide moral support for another young woman who is about to give birth. Lilith leaves the hospital and goes into labor again whilst in the taxi, and this time Frederick is born. Lilith then returns to the hospital where the bewildered Frasier finds his wife and newborn son.

In "My Son, the Father" Lilith states that Zeppo is her favorite Marx brother; she thinks he's the funniest. Ironically, Zeppo is actually the "straight man" who often remains serious amidst his silly brothers. In the "Cheers" bar, Lilith plays the "straight man" as well.

Lilith believes in the idea of classical conditioning and works in a lab with rats, many of whom she forms attachments to. Frasier, however, is a psychoanalyst and believes that many of Freud's theories are valid (Lilith finds Freud sexist and outdated). This leads to many arguments between the two in technical jargon, much to the repeated confusion of everyone else at the bar.

After Lilith discovers that Frasier had a first wife (the famous children's performer Nanny G) that he failed to tell her about, Lilith, predictably, grows very jealous. So, of course, in her usual manner of speech, she proceeds to announce to all at the Cheers bar that she wants to do the above to Nanny G. For once, all who are present appear to be on her side.

Lilith reveals to Frasier that she had an affair with a colleague, Dr. Louis Pescal, and the two are separated in early season eleven. But Lilith comes back to Frasier in "The Bar Manager, The Shrink, His Wife, and Her Lover" and the two are reconciled. However, in the one year gap between "Cheers" and its spin-off "Frasier," they end up getting a divorce.

Bebe Neuwirth had already established herself in theater when she flew out to California to play Lilith. She took the part, thinking it would be a few guest appearances, and was shocked when they asked her on as a series regular. She went on to win two Emmys for her portrayal of Lilith in 1990 and 1991.
