Answer: Meers

One instance of Warren's full name is found in the season 7 episode "Selfless", when Anya's boss D'Hoffryn refers to "the flaying of Warren Meers" as "water cooler vengeance." Wells is Andrew's last name, while Levinson and Rosenberg are the last names of Jonathan and Willow, respectively.
2. What is Andrew's 'claim to fame'?

Answer: He released flying monkeys during a high school play

In "Gone", when Buffy fails to recognize Andrew, he tells her about the flying monkey incident, but she again fails to remember. Andrew's brother Tucker released the hell hounds in season 3's "The Prom", and though Tom Lenk (who plays Andrew) did portray Harmony's henchman Cyrus in season 5's "Real Me", Andrew was never associated with Harmony.
3. When Buffy first meets The Trio in "Gone", which member does she not recognize?

In "Flooded", The Trio hire a demon to rob a bank for them in exchange for the death of the Slayer. The first time we see the team together is in this episode. In "Life Serial", they try to mess with Buffy in various ways (the most memorable one involving a mummy hand). "Gone" is the first episode in which Buffy comes face to face with the team. Finally, "Villains" is the episode in which, at the end, The Trio's self-appointed leader, Warren, meets a gruesome death at the hands of Willow.
5. Which member of The Trio takes the appearances of others on multiple occasions?

Answer: Jonathan

Jonathan takes the guise of a demon in "Life Serial", and that of Warren's deceased ex-girlfriend Katrina in "Dead Things" (which Jonathan refers to as "some of [his] best work").
