Answer: 1999

"Millionaire" ran from August 15 to August 29, 1999. It had 13 episodes.
2. How many questions did a contestant have to answer correctly to win a million dollars in the original format?

Answer: 15

"You are just 15 questions away from winning one million dollars!" Eleven contestants from 1999 to 2003 (including the syndicated version) answered all 15 questions correctly.
3. At two points in the game, a contestant reached a level where they are guaranteed to leave with a certain amount of money. What dollar values were these?

Answer: $1,000 and $32,000

These points were reached at the 5-question mark and the 10-question mark. The formats for this changed much over the years, but the original version was $1,000 and $32,000. "Super Millionaire" and the syndicated version of "Millionaire" had various lock-in points.

Answer: Roughly doubled per question

Eleven questions ($200, $1,000, $2,000, $4,000, $8,000, $16,000, $32,000, $64,000, $250,000, $500,000, and $1,000,000) are worth exactly double that of the question before it.

This is the "time bank" rule, which very rarely came into play. The first time the time bank was used was in the 10th anniversary series (on ABC and with Philbin as host), when a contestant missed the million-dollar question.
