1. "Kingdom Hearts": What popular "Final Fantasy" summon was originally planned to be featured as a summon in this Disney/Square Enix crossover?

Answer: Bahamut

It was planned to include Bahamut, the fan-favorite summon from the "Final Fantasy" series, in the original "Kingdom Hearts". This king of dragons was scrapped in favor of a popular Disney dragon: Mushu. Through hacks, it is possible to have a "Bahamut" option appear on the summon menu, but since it is incomplete it will cause the game to crash. Since the data is still in the game, even some of the first copies that were released had the "Bahamut" option without the use of hacks.
2. All "Kingdom Hearts": "Thinking of you, wherever you are." These are the opening words of Kairi's letter to Sora. In which game do we first see them?

Answer: Kingdom Hearts

When you pop in the disc and go to the load screen, if you don't click anything for a few minutes, a cutscene will play. It shows scenes from "Kingdom Hearts" and the words of Kairi's letter flash on the screen as a foreshadowing of the sequel.

Each Unversed represents an emotion. The only way to tell is by looking at their eyes. The Jellyshade and the Mandrake both represent sadness and the Blue Sea Salt represents anger.

Guardian doesn't seem to be either a Heartless, Nobody, or Unversed but seems to be just a manifestation of darkness. Armor of the Master is an Unversed version of Master Eraqus fought only in the Mentor of Light battle in the Mirage Arena, exclusive to "Birth By Sleep Final Mix". Spirit of the Magic Mirror, surprisingly, is an Unversed that formed from the negativity of the Evil Queen.
5. "Kingdom Hearts II Final Mix+" - Right before the second battle with the Armored Controller in Kingdom Hearts, Sora says something to Riku. What does he say?

Answer: "I didn't say thank you to Namine."

Sora and Riku have a short conversation before taking on Xemnas again (reproduced below):

Sora: "I didn't say it."
Riku: "Huh?"
Sora: "I didn't say thank you to Namine. I never did think it would be easy to say unless everything was set right again."
Riku: "Let's finish this... You'll meet her again."
Sora: "Yeah."

This hints that maybe Sora does in fact remember the events that took place in Castle Oblivion.

Answer: No Heart

No Heart was fought in the Machinations unto Darkness battle of the Mirage Arena, exclusive to "Birth By Sleep Final Mix". Along with his armor, he battles with Xehanort's keyblade, so it can be inferred that this is the same armor that Xehanort had abandoned.
7. "Birth By Sleep": What is the name of the keyblade that the Unknown drops?

Answer: No Name

Spoiler alert! The keyblade definitely reflects how little we know about this tough opponent. Once you defeat the Vanitas Remnant, head over to the Land of Departure summit to battle another tough secret boss! He is a hooded figure with two ray swords that he wields with a reverse grip. Once you defeat him he drops the Keyblade that is named, ironically, No Name.
8. "Birth By Sleep": One of the following worlds was planned to be in "Birth By Sleep". The production team even created the world, and, through some hacks, one can visit this world. Which world was put into "Birth By Sleep" but later taken out?

Answer: "Jungle Book"

A "Jungle Book" world was originally supposed to appear in "Birth By Sleep". Through hacks you can visit King Louie's throne and other memorable locations from Walt Disney's "The Jungle Book".

Mickey will come to aid you if you die during certain battles. The only time he will come to battle Xemnas is during the first Armored Controller battle. No matter how many times you die while battling Demyx, he will never appear. He will only appear when you battle Shan-Yu, Thresholder and Possessor, Shadow Stalker/Dark Thorn, Cerberus, Hydra, Blizzard Lord and Volcano Lord, Storm Rider, Xaldin, Grim Reaper (first battle), and the Armored Controller (first battle).
11. "358/2 Days": What are you sent to eliminate in Mission 31?

Answer: Bully Dog

In Mission 31 Saix sends you to Beast's Castle to defeat the Bully Dog. Xion is your ally.

Answer: True

While the cutscene ends after Xemnas calls the armor "old friend", in another cutscene Xigbar admits to have eavesdropped on Xemnas while he was in the chamber. He makes note that although Xemnas was the only one inside, he heard a second voice.
13. "Birth By Sleep" - Blank Points: During Terra and Xehanort's confrontation, we learn about a second being residing within Terra. Who or what is that second being?

Answer: Master Eraqus

After Terra's body is taken over by Xehanort, both their hearts reside in Terra's body. Terra and Xehanort have a little banter back and forth and it is revealed to Xehanort that Eraqus' heart found its way into Terra's.

In "Kingdom Hearts Final Mix", there is a new cutscene on Destiny Islands during the second day. Kairi reveals that the good luck charm she gives Sora towards the end of the game is the same one she was making when they were getting ready to leave the island.
