1. Who are the two traitors at the beginning of the play?

Answer: Macdonwald and the Thane of Cawdor

At the beginning of the play, two traitors are introduced as Macdonwald (yes, that's the spelling) and the Thane of Cawdor. After Macbeth shows true loyalty in the field of battle, he gains the title of "Thane of Cawdor" after the original thane is killed.
Act I, Scene II, Line 11-Captain: "The merciless Macdonwald (Worthy to be a rebel,..."
Act I, Scene III, Line 60-Ross: "Assisted by that most disloyal traitor The Thane of Cawdor,..."
2. Where does Malcolm flee after his father's death?

Answer: England

King Duncan's sons, Malcolm and Donalbain, are afraid for their own lives after their father's death. Malcolm flees to England, and Donalbain goes to Ireland.
Act II, Scene III, Line 160-Malcolm: "Which the false man does easy. I'll to England." Donalbain:"To Ireland I. Our separated fortune shall keep us both the safer."
3. Why doesn't Lady Macbeth kill King Duncan herself?

Answer: She says he resembles her father

Lady Macbeth can not kill the King herself because he resembles her own father while he was sleeping.
Act II, Scene II, Line 16-Lady Macbeth: "Had he not resembled my father as he slept, I had done't."
4. What was the second apparition?

Answer: A bloody infant

The first apparition was an armed head. It told Macbeth to beware Macduff, the Thane of Fife. The second apparition was a bloody child. It said no man born of woman shall harm Macbeth. The third appariton was a crowned child holding a tree in his hand. It warned Macbeth that he shall not be killed until Birnam Wood comes to Dunsinane Hill.
Act IV, Scene I.
5. Who was Siward?

Answer: Earl of Northumberland

Siward was Earl of Northumberland. He was head of the 10,000 English forces brought against Macbeth.
6. Who was Queen of the Witches?

Answer: Hecate

Hecate was superior to the three witches. She scolds them for getting involved with Macbeth, and warning him of his future.
Act III, Scene V, Line 3-Hecate: "How did you dare to trade traffic with Macbeth in riddles and affairs of death; and I, the mistress of your charms..."
7. Macbeth hires murderers to do his dirty work for him. Who do they kill?

Answer: Banquo

The murderers were supposed to kill both Banquo and his son Fleance; however, Fleance got away in the dark.
Act III, Scene IV, Line 29-Murderer: "There's but one down; the son is fled."
8. Who said this? "Out, damned spot! out, I say!"

Answer: Lady Macbeth

Lady Macbeth says this in Act V, Scene I while she is sleepwalking.
