Shirts used to be made in standard sizes with separate collars and cuffs. The bands were to adjust the lengths of the sleeves. Since card dealers and bartenders worked with their jackets off the bands were always visible and the tradition carried on.

The sleeve band showed that the dealer had nothing up his sleeve, and card rooms usually had overhead lighting and the visor kept the light out of his eye so he could see the cards.
