Her tumor is getting larger. When Tom comes home one evening, Izzi is outside without a jacket or shoes on, standing in the snow, looking at the stars. Later while she is bathing, she asks him to add hot water to the tub. It is then that Tom realizes that something is very wrong and she admits to him that she has been losing sensation - especially sensitivity to hot and cold - for weeks.

Tom works at a research laboratory. Donovan is a simian who has a brain tumor and Tom is trying to develop a cure for this type of cancer. Donovan is injected with the experimental serum and at first it appears to have no affect on the tumor. Donovan reacts well to the medicine and his brain activity increases tremendously but the tumor doesn't initially appear to be shrinking.

However, days later it is discovered that the serum is working and Donovan's tumor starts to shrink. Could it be that Tom has found the cure for cancer?

When Tom scrubs for surgery, he always takes the ring off and places it just above the sink on a shelf. However, when he gets out of surgery, the ring is missing and remains lost for the remainder of the movie. The ring is symbolic to him as it represents a promise he made to Izzi, to be with her forever.

It is a bit of foreshadowing when he does lose it, showing how fragile and short his time with his wife is.

Izzi is showing Tom an ancient Mayan book that details the story of a man who sacrificed himself and a tree grew from his body, essentially giving him eternal life. Tom is upset when Izzi talks about death and goes to leave but when he turns around, he notices that she is on the brink of collapsing.

He catches her unconscious body and she is rushed to the hospital. Her brain tumor has grown and caused a seizure. Tom, unwilling to let her go, begs her to hang on a little while longer as they are working on a cure at the laboratory. Izzi tells him that she doesn't fear death anymore but he doesn't want to listen - he doesn't want to lose his true love.

Izzi makes parallels to her marriage with Tom and the two characters in her story. Tomas is the Conquistador who offers to defend Spain to his death and Queen Isabel, knowing that his heart is true, asks him to find the Tree of Life. If he is successful in his task, the Queen offers herself to him so that they may rule Spain together forever.

Tomas' journey takes him to a secret and ancient civilization. He finds the Tree of Life, but its power is strong and unexpected.

Izzi asks Tom to complete the final chapter of her story, "The Fountain", using the set she gives him in the hospital. After Izzi dies, Tom accidentally knocks the bottle of ink over and spills it. Distraught over the loss of his wife, he takes the pen and ink and essentially tattoos his wedding finger with a ring that will stay with him forever.

The Mayan King had stabbed Tomas in the stomach and when he puts the white sap on the wound, it immediately heals. Seeing its power, Tomas drinks it but the sap doesn't make him live forever. It causes plants to sprout from his body and ultimately consumes and kills him.

She dies from the tumor. Moments after her death, Tom learns from Lillian that the serum they have been developing is working and Donovan's brain tumor is shrinking.

Izzi tells Tom the story of the man who, after he died, had a tree grow from his body, essentially allowing him to live forever. She repeats this quote to Tom, hoping that her death will also lead her to her own awe.

Part of the movie focuses around Tom as a futuristic astronaut, making his way to the nebula that Izzi had spotted. While the tree is alive, he eats small pieces of its bark but when Izzi dies, the tree dies as well. No longer will the two of them live together forever.
