Ellen died in an explosion in "Abandon All Hope". Ellen and Jo and the Winchester brothers were pinned down by hellhounds and Jo was already dying. Ellen sacrificed herself to stay with her daughter and kill the hellhounds so the brothers could get away and go after Lucifer.

He would also come back in the episode Hunted and try to kill Sam after finding out he was supposed to turn evil. In the end he was finally arrested.

But he injured his left shoulder in the sewer, so he wouldn't have been able to move his arm so much. Sam then walks around the car and points his gun at Shapeshifter Dean. While talking, Shapeshifter Dean hits Sam, knocking him out.
