During the 2021 women's Royal Rumble Match, the reigning WWE 24/7 Champion R-Truth randomly ran to the ringside area. A mob of title challengers then began chasing R-Truth, so he entered the ring as the women's Royal Rumble Match was still being competed. Competitor Alicia Fox pinned R-Truth to become new WWE 24/7 Champion. Once Alicia Fox was eliminated from the match, R-Truth immediately pinned her to regain the WWE 24/7 Championship.

The reigning WWE 24/7 Champion R-Truth did appear during the 2021 men's Royal Rumble Match.

Damien Priest lasted over fifteen minutes in the match. He eliminated four other competitors. He was eliminated from the match by Bobby Lashley.

The Miz and John Morrison had a backstage interaction with musician Bad Bunny. Bad Bunny didn't want to hang out with The Miz and John Morrison so the tag team left his dressing room.

When The Miz entered the match, he destroyed Bad Bunny's DJ equipment on his way to the ring. Bad Bunny came down to the ring to distract The Miz and John Morrison. The distraction caused both members of the tag team to be eliminated from the match. Bad Bunny then climbed the turnbuckle performed a cross body on the tag team.

Jeff Hardy from the Hardy Boyz entered the the match at number five. Jeff Hardy's former tag team partner, Matt Hardy, did not enter the match.

Members of past tag teams Rated RKO (Edge and Randy Orton), the Bar (Sheamus and Cesaro), Team Hell No (Daniel Bryan and Kane) and the New Day (Xavier Woods and Big E) drew consecutive entrance numbers in the match. Former tag team partners Edge and Christian were in the match together as well.

Kofi Kingston was suffering from a jaw injury on the night of the 2021 WWE's "Royal Rumble" event. During a backstage segment at the event, Kingston told Xavier Woods and Big E that he wasn't medically cleared to compete that night. Thus Kofi Kingston wasn't able to perform a creative stunt in the 2021 men's Royal Rumble Match. Kofi Kingston also showed off his "Luke Harper" themed shirt during the backstage segment.

Omos began serving as a bodyguard for AJ Styles in October of 2020. Omos accompanied AJ Styles to the ring for this match. During the the match, Omos physically prevented Styles from being eliminated. Omos eliminated Big E and Rey Mysterio from the match even though he wasn't a participant.

Braun Strowman returned from a "suspension" the weekend of the 2021 WWE's "Royal Rumble" event. Braun Strowman entered the match at number thirty. Strowman lasted over seven minutes and eliminated three other competitors. Strowman was eliminated from the match by a group of three competitors.

After Edge eliminated Seth Rollins, Edge received a surprising "RKO" from a returning Randy Orton. Edge recovered to eliminate Randy Orton to win the match.

Edge entered the 2021 Royal Rumble Match at entry number one. Edge eliminated three participants and lasted over fifty-eight minutes in the match.
