Answer: Connie and Steven

Stevonnie is a huge deal because before them, it was believed that Gems and humans could not fuse at all. They have the appearance of one that is Steven and Connie's combined ages. Stevonnie is voiced by A.J. Michalka and first appears in the "Alone Together" episode.

Answer: Rainbow Quartz

In the episode "We Need To Talk," Rainbow Quartz made an appearance in the music video for Greg and Rose Quartz's duet "What Can I Do For You." Pearl told Rose Quartz she thought it would be a great idea for the video, to make it look more interesting.

But what Pearl really wanted was to ignite a curiosity in Greg about fusion and how it worked before stomping on his hopes by pointing out yet another thing that he did not have in common with the Gems. And therefore, one more reason why he and Rose should not be together.

Answer: Amethyst and Garnet

Sugilite made her debut in "Coach Steven." She is an unstable, reckless fusion with a flail for a weapon. Strongly resembling Kali, the Hindu goddess of destruction, Sugilite's goal seem to be related to the same (and impressing Steven). She is the only fusion known to have five eyes (as she is technically a fusion of THREE gems)...

Answer: the Gems performing a synchronized dance

Each of the Crystal Gems' fighting styles and, by association, fusion dances is based on specific dance styles. Pearl's style is ballet, Garnet's is waacking (created in the 1970s Disco era by the LGBT community in Los Angeles -- look it up), Amethyst's is dancehall/club dancing, and Steven's is just...Steven.
At NY Comic Con 2016 when asked why some fusions can occur without much/prolonged dancing, Kat Morris (Storyboard Supervisor and outline writer for "SU") said that fusion is "more than the dance, it's about a mutual understanding."

Fluorite first appears in "Off Colors," where she was hiding in the Abandoned Homeworld Garden with Rhodonite, Padparadscha, and the Rutile Twins before they were found by Lars and Steven. Her appearance is similar to that of a caterpillar (like the one from "Alice in Wonderland") and her colors range from a seafoam green to a purplish-pink. Her gems are unknown.

Answer: a forced fusion

While we can agree that The Cluster is an unstable mess (theoretically-speaking), they are considered a forced fusion. They didn't decide to be smashed together deep within the center of the Earth.

Luckily for us, they have decided to work together to protect the Earth as it is their home, too.

Answer: No

Ah, Rose Quartz.
So Rose isn't a fusion, but she isn't just any old Gem either. She's bigger than the other Crystal Gems because she is Pink Diamond. It is believed that she is smaller than the other Diamonds because she doesn't have as many colonies to draw energy from.

Answer: a recurve bow

Opal's weapon consists of Pearl's spear as the bow, and Amethyst's whip as the string. Opal's arrows also take on a shape similar to Pearl's spear. Smoky Quartz's weapon is the yo-yo, made from Steven's shield and Amethyst's whip. The warhammer is Sardonyx's -- Garnet's gauntlets on either side of Pearl's spear. And the flail is Sugilite's. Garnet's gauntlets folded together, and attached to Amethyst's whip. Steven refers to it as a "wrecking-ball-thingy," which it kind of is.

Answer: Alexandrite

Alexandrite, also known as Mrs. Universe, has six arms. She is the fire-breathing fusion of Amethyst, Garnet, and Pearl. Fluorite only has four arms (and six feet!). Sardonyx has four, and Smoky Quartz has three.

Answer: Garnet

Garnet is, in a way, the love child of Ruby and Sapphire. They're married now, and through their official union -- after being together for 5,750 years -- we have Garnet again.

We could say that Steven is somewhat of a fusion, as far as Gems mating with humans can go?
Topaz is a perma-fusion out of necessity. She is fused because that's how she exists to complete her tasks.
Malachite is in no way a healthy fusion. She is a fantastic example of an unstable fusion (and if we want to get serious, an abusive relationship).
