Sheehan, the "elusive" doctor, is actually in plain sight of Teddy for most of the four days at Shutter Island. He is disguised as Chuck Aule, the partner sent with him on his investigation.

Andrew created Rachel Solando, Teddy Daniels, and the fire in the apartment building to block from his mind that it was his fault his children and wife were dead.

Although he realizes, briefly, the truth of his actual identity, the final chapter of the book reveals a repeat of the story his mind has created and believed for two years, with the suggestion that since Cawley's mission to save him failed, Teddy will go into surgery for a transorbital lobotomy.
