1. How many Forsaken are there?

Answer: 13

There are 13 Forsaken. Their names are Aginor, Asmodean, Balthamel, Be'lal, Demandred, Graendal, Ishamael, Lanfear, Mesaana, Moghedien, Rahvin, Sammael, and Semirhage. They were 13 of the most powerful Aes Sedai in the Age of Legends who went over to the Dark One.
2. Which Forsaken does Moridin have a cor'sourva for?

Answer: Moghedien

A cor'sourva is a soul trap, in the Old Tongue. If you press even slightly on it, the person who is in control of the cor'sourva will be under your control for the rest of their life, and they have no will power or free choice.
3. Which ash'aman is Osan'gar masquerading as?

Answer: Dashiva

Corlan Dashiva, who is strong in the One Power and very distant, is actually Osan'gar. He is killed by Elza Aes Sedai, who is linked with Eben, an ash'aman, when Rand al'Thor, the Dragon Reborn and Nynaeve al'Meara, Aes Sedai of the Yellow Ajah, are cleansing saidin.
4. Where is the prison of the Dark One and the Forsaken?

Answer: Shayol Ghul

The Dark One and the Forsaken were sealed into a prison at Shayol Ghul, which is a mountain in the Blasted Lands. It was sealed with seven seals, each a circle with the ancient symbol of the Aes Sedai on it. Four have been found, and only three are holding at the end of "Winter's Heart". The Forsaken walk free, and the Dark One is free enough to touch the world.
5. Ishamael was killed in "A Crown of Swords".

Answer: False

Sammael, posing as Lord Brend, of Illian, was killed by Rand al'Thor in Shadar Logoth. Ishamael is killed by Rand at Falme, when he poses as the Dark One. But first, he gives Rand his never-healing wound. The battle between the Dragon Reborn and the Forsaken is in the sky, and it is after that battle that Rand proclaims himself the Dragon.
6. Who apparently kills Lanfear?

Answer: Moiraine Damodred, Aes Sedai of the Blue Ajah

Moiraine, battling Lanfear in an attempt to save Rand al'Thor's life, threw herself and the Forsaken into a ter'angreal in Cairhien, apparently killing both of them.
7. What is the name for the Forsaken amongst themselves?

Answer: Chosen

The Forsaken call themselves the Chosen, for they are the 13 of most powerful Aes Sedai ever known. Only Nynaeve al'Meara can defeat one of them (Moghedien), and she is one of the lesser Chosen.
8. Which high-ranking Aes Sedai is a Dark Friend in the Black Ajah and is in the service of Mesaana?

Answer: Alviarin, the Keeper

Alviarin, who has Elaida wrapped around her finger, is in the service of Mesaana. She wrote a coy letter to the Dragon Reborn, who considers Alviarin an ally in the White Tower. Could Rand be making a big mistake?

Answer: Moridin

Moridin (in the Old Tongue, death) could be a new or old Forsaken. His status among them is unclear.

Balthamel is the first Forsaken to die. He is killed by the Green Man at the Eye of the World. Moiraine attacks Aginor, but barely leaves a mark.
