Answer: Two score years

Forty years have passed since the un-believer's last visit. In the first volume Lord Foul prophesied that he would be in Revelstone in "7 x 7 years". Although not yet at the end of this time period, Lord Foul has developed a large army to ravish the Land. In desperation, the help of Thomas Covenant, wielder of the white gold, has been sought.
2. When the un-believer was previously in the Land, the High Lord was Lord Prothall. Who is the current High Lord?

Answer: Elena

Elena is the product of the rape of her mother, Lena, by Thomas Covenant in the first Chronicles. The relationship between the two is a significant thread in this novel, the ur-Lord filled with self-loathing and Elena filled with love. These feelings and the extent to which others know of the true relationship between the two is also an major theme throughout this book.
3. At Revelstone, Covenant meets another person from 'his' world: Hile Troy, who is the warmark. What disadvantage does he have that would seem to prevent him being effective as a war leader?

Answer: He is blind.

Blindness would seem to be tricky when in command of soldiers. However, using the curing powers of the Land, Hile Troy is able to see although he still has no eyes, just empty eye sockets. In the world that both Thomas Covenant and Hile Troy come from, Hile Troy was on the ninth floor of a burning building and is presumably dead in that world.
4. By using the corrupting powers of the Illearth stone, which of the Lords' allies has Lord Foul almost annihilated?

Answer: Giants

Lord Foul's three most powerful servants, the ravers, have possessed three giants and slaughtered all other giants found at Seareach. Only those giants elsewhere survived the massacre.
The Bloodguard are still true to the Lords, although towards the end those that picked up a fragment of the Illearth Stone also become vulnerable to the power of Lord Foul.

The Illearth stone, the cause of the ill earth, is the source of the book's title.
5. The being Amok appears in Revelstone, much to the consternation of the Lords since he arrived undetected. What does Amok turn out to be a guide to?

Answer: The Seventh Ward

Amok is a creation of the High Lord Kevin. It would seem he has appeared at the wrong time, as the Lords have only recovered the second ward so far. This in turn is caused by the actions of the unbeliever, who manages to trigger his appearance unintentionally.

Amok reminds me of an annoying computer game. Unless the right questions are asked, the responses given are meaningless and infuriating.
Of the other options, the Staff of law was recovered in the first book of the chronicles, "Lord Foul's Bane". The Arch of Time has the function of limiting the power of both the Creator and the Despisor in the Land. The Vortex of Trepidation appears as a creation to dishearten the defenders later in the story.
6. Lord Shetra and Bloodguard Cerrin are killed in the Sarangrave Flat. What creature or creatures killed them?

Answer: The Lurker

Sarangrave Flat is a bog, a place of rivers and quicksands. Lord Foul has used the powers of the Illearth Stone to awaken it. Although it only appears in a small scene in this book, the Lurker reappears in the second and third chronicles as a significant and more detailed creature.

In "The Illearth Stone" only tentacles of it are seen, which try to pull people into the foul swamp waters (and sometimes succeed).

Answer: Kevin Landwaster

The creation Amok guides Covenant and High Lord Elena along with their Bloodguard to the the location of the Seventh ward. Through the ur-lords' careful question of the bloodguard, the seventh ward is uncovered. But using this Elena summons the shade of Kevin Landwaster to ask him how to defeat Lord Foul. Unfortunately this breaks the Law of Death. As a consequence, Lord Foul uses the Illearth Stone to control the shade, and commands it to kill the High Lord.

It could be argued that Lord Foul killed Elena, or that the un-believer, Amok or even the bloodguard had their roles that led to the final result, but the death is directly caused by the shade of Kevin Landwaster.

Answer: The Raver Jehannum, also known as Fleshharrower

The Ravers are the most powerful of Lord Foul's servants. They are spirits that can possess and control less powerful creatures and, with the additional powers of Lord Foul, they are able to possess more powerful creatures. Although their physical forms can be destroyed, their spirits remain. In "The Illearth War" they are all in the bodies of giants, with all the strength and powers of such creatures.
It would seem that they cannot just move from one physical body to another at will, as Jehannum would clearly depart from the giant's body before it is hung at the end of "The Illearth War".

Answer: Garroting Deep

Caerroil Wildwood, Forestal of Garroting Deep, has no time for men, even the Lords, since the devastation of the forests by the Ritual of Desecration. However, he is tempted by the chance to kill the physical body of a raver. As such he brings the forest to life to destroy the army of Lord Foul.
This seems to me to be an echo of Tolkien and the Ents of Fangthorn forest.
The other options are all places in "The Land".

Answer: Hile Troy

On agreeing to let the army of the Lords pass through and destroy that of Lord Foul, the forestall requires an unspecified bargain. Before it is stated, and before Lord Mohoram can stop him, Hile Troy agrees to it to protect his army.
After the battle Hile Troy is turned into the form of a tree by the song of Caerroil Wildwood, becoming the Forestal's apprentice Caer-Caveral.
