1. Do all the actors who portrayed the nine members of the Fellowship suffer an injury/injuries?

Answer: No

While there were a lot of injuries during the filming of the trilogy suffered by both cast, crew, and especially stunt people - it is usually injuries by the members of the fellowship, and those they come in close contact to, that is reported by the press and talked about in the special features part of the DVD. Out of the Fellowship, Elijah Wood (Frodo Baggins), John Rhys Davies (Gimli), and Sir Ian McKellen (Gandalf) don't sustain any injuries.

At least, none reported in the DVDs of the theatrical versions.
2. Speaking of injuries, what injury does Sean Astin (Samwise "Sam" Gamgee) obtain while filming "FOTR"? (remember, injury meaning he got hurt)

Answer: his left foot is pierced by a shard of glass

It's during the scene in 'FOTR', at the end, when Frodo has taken a departed the Fellowship and taken a boat. Sam catches up with him and wades in the water to follow and accompany Frodo on his journey to Mount Doom. During one of the shots, Sean walked out into the water then suddenly froze, he didn't yell or anything, and everyone on the set knew that he was injured and that it was serious.

When he walked out, it turned out that he had stepped on a shard of glass that the divers apparently missed while they were searching the bottom for dangerous objects.

He needed 20 stitches for it and it was a very painful experience for him, yet 24 hours later, he was back on set, continuing to film the trilogy.
3. What injury does Viggo Mortensen (Aragorn) get?

Answer: he broke 2 of his toes

Actually, he got two different injuries, if a chipped tooth he got when he received a blow on the face by a prop sword can be counted as an injury. In the scene when Legolas, Aragorn, and Gimli are in the piled carasses of the Uruks that the Rohirrim slaughtered in the night and looking for remnants of the Hobbits Pippin and Merry, Aragorn yells his grief and frustration. Peter Jackson, the director, had him do the shot where he kicks the helmet again and again because Pete wanted to shoot Viggo kicking the helmet towards the camera and flying above and beyond it.

After so many takes, Viggo yells, kicks the helmet, yells again and drops to his knees and everyone's wondering why he did that. Apparently, it was because he had broken his toes. Pete decided to keep that shot because he felt that the "grief" was heartfelt.
4. Does Dominic Monaghan (Meriadoc "Merry" Brandybuck) get hurt, mild or otherwise?

Answer: yes

In 'FOTR', when the four Hobbits are running away from the Black Riders and they all get on a raft to cross the river, Dom gets a very painful splinter. Although a very mild injury compared to Orlando's broken rib, Viggo's broken toe, and Sean's pierced foot, Dom put up a fuss with the splinter pain. From then on, Billy Boyd (Pippin) would always tease him whenever Dom looked sweaty and in pain, "Is it a splinter again?"
5. In all three movies, how many times do we see the One Ring worn on Frodo's finger?

Answer: 4

He first "wears" it in the Village of Bree, in the Inn of the Prancing Pony, while waiting for Gandalf. Remember that scene? He slipped, tossed the Ring, and the Ring fell, slipping right into his index finger. Later, he wears it in Weathertop when the five Ringwraiths are after him and he sees what they look like in the Shadow World.

He wears it again at the end of the first movie, in Amon Hen, while trying to get away from Boromir (thanks to IdrankWHAT for reminding me!) He doesn't wear it at all during 'TTT' but wears it again in 'ROTK' when he's in Mount Doom.

Instead of being able to throw it down to the lava flowing down below, he finally gives in to the Ring's pull and wears it. It's the last time he, or anybody else, gets to wear it since Gollum bites his finger off, they fight, and Gollum falls down with the Ring and dies.
6. Throughout the entire trilogy, how many times does the One Ring grace the finger of its current owners?

Answer: 10

Sauron obviously wears it because he's the maker and original owner of the Ring. Next owner is Isildur and he wears it when he's lying face-down in the river where the Ring abandons him and he gets shot by 3 arrows in the back and dies. Then Smeagol/Gollum (shown in a flashback scene in 'FOTR' & 'ROTK'), Bilbo (at his 111th birthday party), and Frodo wears it four times: at the Prancing Pony, Weathertop, Amon Hen, and Mount Doom.
Sam puts it on twice after he thinks Frodo is dead so he is, technically, the owner of the rig for that brief period.

Answer: Isengard

Isengard is indeed in all three movies. In 'FOTR' and 'TTT', Isengard is often shown because Saruman is one of the secondary main characters. In 'ROTK', Isengard is shown at the start, when we see what Aragorn, Legolas, Gimli, and Gandalf are up to. They see Pippin and Merry standing at the edge of Isengard, they enter, and they talk to Treebeard.

The shire is in the first and third movies, Fangorn forest in the second and third, Lothlorien is only in the first. Unless you'd like to count Galadriel's scenes (you only see her face, not where she is) in 'TTT' where she talks to Elrond about whether or not to give aid to the humans against their battle with Sauron.

Answer: Edoras

Rivendell is always shown - scenes of/with Lord Elrond and Arwen are located here. Mordor is, of course, in all movies because it's an important location and they always pan there whenever updates on Sauron is exposited. Probably not that noticeable, Minis Tirith is also found in all three movies; in 'FOTR', Gandalf goes here to find out about the One Ring, in 'TTT' you can see it in the background as Faramir heads off to Osgiliath, and 'ROTK' takes place mostly in Minas Tirith. We only see Edoras in the second and third movies.
9. What injury did Orlando Bloom (Legolas) get?

Answer: broken rib

He broke a rib when he fell off a horse and Gimli's scale double fell on top of him. It was painful but he still worked regardless. In fact, all the shots where they show Aragorn, Legolas, and Gimli running around after the Uruks (first part of 'TTT') they are all injured. Legolas the broken rib, Aragorn the broken toes, and Gimli's scale double (Brent) the broken knee. And nobody broke an arm during filming.
10. From where did Andy Serkis get his inspiration for his character's trademark "Gollum, Gollum" cough?

Answer: a cat coughing up a furball

It does sound remarkably like a cat coughing up furballs, doesn't it?

Answer: ('TTT') the scene where Gollum is catching a fish in the river

Andy Serkis said it was the hardest. You see, the scene was supposed to take place in summer, but in reality, they had to film during winter and it snowed the night before they were scheduled to shoot. They didn't have enough budgeted time to pick another time to shoot with the place, so Pete said that they had to melt the snow at one part of the river so that they could shoot. Well, the snow melted all right, but that doesn't mean the water is warm.

It was freezing and Andy got an incredible shock when he first hit the water.

He also had to do it a couple of times, but he was game and did what Pete wanted, freezing water notwithstanding.

They used a quarry and built a set for Helm's Deep there, after they finished shooting all Helm's Deep scenes, they tore down the set and built the set for Minas Tirith there.

He's left-handed. Althought weirdly enough, when he hits the spears of his men in 'ROTK' just before attacking in Pelennor Fields, he's using his right hand.

Even on his days off (and they rarely had those), Viggo Mortensen would ride his horse and practice his horseback riding. He was very insistent on doing all his horseback riding stunts.

They added it because Viggo insisted on it and even practiced it with his horse to show that it could be done.
