According to The Internet Movie Database, Rod Serling is uncredited for this role, but he did in fact provide the voice over. Serling's most famous role was that of narrator for the tv series "The Twilight Zone".
2. What name completes the title of the opening song: "Goodbye, ______, Goodbye?"

Answer: Eddie

This song opens the movie, and it tells the tale of a young singer who commits suicide, so his memorial album will garner enough money for his younger sister's much needed operation. This song is important because it opens the movie, and at the very end of the song, we get our first glimpse of the mysterious Swan.
3. What is the name of the fictitious nostalgia band that performs at the movie's open?

Answer: The Juicy Fruits

The band is revealed as The Juicy Fruits when we see Winslow putting a banner reading "Winslow Leach at the Piano" over a giant poster that says "Juicy Fruits Performing Live Tonight". Archie Hahn, Jeffrey Comanor, and Harold Oblong portray The Juicy Fruits. They also appear in the movie as members of the ficticious bands The Undeads and The Beach Bums.
4. What famous songwriter/actor plays the mysterious Swan?

Answer: Paul Williams

Paul Williams has written many famous songs such as "We've Only Just Begun", which was a hit for The Carpenters, and the famous Muppet song "The Rainbow Connection". He also wrote all of the music for the movie.
5. What is the name of the young singer who the Phantom is obsessed with?

Answer: Phoenix

Phoenix is a beautiful young singer who is determined to do anything to make it big. She even goes so far as to sleep with Swan, much to the Phantom's chagrin. Phoenix is portrayed by Jessica Harper. This very talented actress also played the role of Janet Majors in the cult favorite "Shock Treatment".
6. What is the actual name of The Phantom?

Answer: Winslow Leach

Winslow Leach, played by William Finley, assumes the identity of the Phantom after Swan robs him of his music, frames him, sends him to jail, and indirectly disfigures him.
7. After being framed, does winslow get sent to Sing Sing prison?

Answer: Yes

This is ironic in a way, seeing as Winslow is a songwriter. During his stay at Sing Sing, Winslow is part of an experiment that involves the removal of all his teeth, as they are "a source of infection". He is also forced to put together children's board games until he manages to escape by choking a guard, and climbling into a toy box, which then falls off the delivery truck.
8. What is the name of the glam rock star who literally gets fried onstage by The Phantom?

Answer: Beef

Beef, played by Gerrit Graham, is an effeminate rock star who takes over the singing of The Phantom's cantata after Swan decides that Phoenix is far too perfect to sing it. The Phantom kills him after Beef ignores his warning that if anyone besides Phoenix dares to sing his cantata they WILL die.
9. What is the name of the song that Phoenix sings when she finally take the stage at The Paradise?

Answer: Old Souls

"Old Souls" is a sad, beautful song about star-crossed lovers who meet again and again during the course of many lifetimes. "Special To Me" is what Phoenix sings when she first auditions, "Life At Last" is what Beef sings shortly before he is killed, and "Somebody Super Like You" is sung by The Undeads before Beef takes the stage.
10. What song plays over the closing credits of the film?

Answer: The Hell Of It

This song speaks of going to a musician's funeral to "say goodbye". It also speaks about how the musician is "better off dead" and how "Although [the musician's ] music lingers on, all of [the people at the funeral] are glad [the musician's] gone". This song, which plays over clips from the movie, can be applied to most of the main characters.
