1. The 1st Doctor (William Hartnell)

Answer: Dodo Chaplet

Dorothea "Dodo" Chaplet (played by Jackie Lane) traveled with the first Doctor in the third season of the show, making her first appearance at the end of "The Massacre of St Bartholomew's Eve" after the TARDIS returned to 1966 from 1572 France. She wandered into the TARDIS, believing it to be an actual police box, in order to report an accident.

Altogether, Dodo Chaplet appeared in six stories, leaving partway through "The War Machines" for a rest in the country. She never returned, purportedly deciding to remain in the 20th century.
2. The 2nd Doctor (Patrick Troughton)

Answer: Jamie McCrimmon

James Robert "Jamie" McCrimmon (played by Frazer Hines) was a piper from Scotland in 1746 that the second Doctor (and companions Ben and Polly) met in the serial "The Highlanders" in the fourth season. He joined them at the end of the story and continued to travel with the Doctor until his regeneration into the third Doctor (Jon Pertwee).

When the Doctor was put on trial and exiled to Earth by the Time Lords, his companions Jamie and Zoe were returned to their own times with their memories of the Doctor wiped.
3. The 3rd Doctor (Jon Pertwee)

Answer: Jo Grant

Jo Grant (played by Katy Manning) joined the third Doctor at the beginning of the eighth season (in "Terror of the Autons") as a UNIT-appointed replacement for previous companion Liz Shaw. Her tenure with the Doctor lasted a full three seasons and she made her departure at the end of "The Green Death" on amicable terms, falling in love with environmental scientist Professor Clifford Jones (Stewart Bevan) and choosing to go with him to the Amazon.
4. The 4th Doctor (Tom Baker)

Answer: Leela

Leela (played by Louise Jameson) came to the fourth Doctor in the 14th season (in "The Face of Evil") from an unspecified time far into the future as a savage warrior woman. Her tribe, it turned out, were the descendants of a survey team from a crashed Earth ship. Leela traveled with the Doctor until the end of the 15th season during "The Invasion of Time" serial.

She chose to stay with Andred (Chris Tranchell), a commander in the Chancellory Guard of Gallifrey, and K-9 (Mark I) stayed with her as well.
5. The 5th Doctor (Peter Davison)

Answer: Vislor Turlough

Vislor Turlough (played by Mark Strickson) joined the fifth Doctor under suspicious circumstances in "Mawdryn Undead", one of the serials within the "Black Guardian" trilogy in season 20. In fact, Turlough had been commissioned by the Black Guardian to kill the Doctor! In the end, however, his loyalties changed, and he continued to travel with the Doctor until the end of the 21st season, leaving at the conclusion of "Planet of Fire" when he learned that he could return to his home planet (Trion) from political exile.
6. The 7th Doctor (Sylvester McCoy)

Answer: Ace

Ace (played by Sophie Aldred) joined the seventh Doctor at the end of season 24 in the serial "Dragonfire". Although from 1980s Earth, the Doctor (and companion Mel Bush) found her on the planet Svartos... she had been swept up in a time storm and deposited there. Ace proved to be the last companion of the Doctor in the original series, as it was cancelled at the end of its 26th season.

Her departure from the Doctor was never revealed (on television), as she was still traveling with him when the show ended. Ace was no longer with the seventh Doctor when he appeared at the beginning of the 1996 "Doctor Who" movie.
7. The 8th Doctor (Paul McGann)

Answer: Grace Holloway

Grace Holloway (played by Daphne Ashbrook) was the ER doctor called in to operate on the seventh Doctor after he was shot at the opening of the 1996 "Doctor Who" movie. Unknowingly, she inadvertently caused his death while trying to save him, believing him to be a normal human. With a faulty memory and no one else to turn to, the newly-regenerated eighth Doctor found Grace and foiled the plans of the Master (Eric Roberts).

In the end, Grace chose not to fly away in the TARDIS with the Doctor.
8. The 10th Doctor (David Tennant)

Answer: Martha Jones

Martha Jones (played by Freema Agyeman) joined the 10th Doctor for series 3 of the revived show, beginning with the first story "Smith and Jones". She chose to leave the Doctor after the events of the series finale, "Last of the Time Lords", rather than stay in an unrequited relationship.

While she was the Doctor's companion for only that one series, Martha made a number of followup guest appearances in both "Torchwood" and "Doctor Who", making her final appearance in the special "The End of Time" when the Doctor made his 'farewell tour' before regenerating.
9. The 11th Doctor (Matt Smith)

Answer: Amy Pond

Amy Pond (played by Karen Gillan) made her first appearance alongside the newly regenerated 11th Doctor in his first story ("The Eleventh Hour") of series 5 of the revived show. Her fiery character traveled with the Doctor until midway through series 7 when she chooses to follow her husband Rory to a certain, unchangeable fate (in "The Angels Take Manhattan").

Amy made one more cameo appearance in "The Time of the Doctor" for the 11th Doctor's death/regeneration scene.
10. The 12th Doctor (Peter Capaldi)

Answer: Nardole

Nardole (played by Matt Lucas) first appeared in the 2015 Christmas special "The Husbands of River Song" as River's servant (in the year 5343), and stayed on for the 2016 Christmas Special and series 10. Nardole holds the distinction of being the first alien companion of the revived series, and the first since Vislor Turlough (who, need I remind you, traveled with the fifth Doctor).
