"Survival" aired on December 6, 1989 and "Rose" aired on March 26, 2005.

2. When the Doctor regenerated into Jodie Whittaker's Doctor, how many times had the Doctor regenerated?

The Doctor has regenerated into 15 different versions, although not all were in the traditional sense.
Between Doctors Eight and Nine (Paul McGann and Christopher Eccleston) there is what was called the War Doctor (John Hurt) that was first introduced in "The Name of the Doctor" and featured mostly in the episode "The Day of the Doctor", the show's 50th Anniversary special. Although not shown much, he is still the Doctor.
During the episode "The Stolen Earth" the Doctor is fatally wounded and- not wanting to change- channels his regeneration energy into a spare hand he had thus creating what we would call TenToo (or the Metacrisis Doctor).
