1. Max is how tall and weighs how much?

Answer: He is 5'9" tall, weighs 150 pounds

Though in the wedding episode, "With love And Twitches", he says seemingly without irony that he's 5'11", on several occasions prior he states that he's 5'9", and that's the one we die hard "Get Smart" fans go with. Always throughout the series he is said to be 150 pounds.
2. What year was Max born in, and what is his Zodiac sign?

Answer: Born in 1930, Zodiac sign is Scorpio

The tombstone in the season five episode, "The Mess of Adrian Listenger", is marked with the year 1930, and in the season two episode, "The Decoy", when Max is taken by "the old gas bomb in the horoscope book trick!" he is looking for the sign Scorpio.
3. What doctor delivered Max?

Answer: Dr. Linquist

We learn this in the season five, in "And Only Two Nintey-Nine". The lines were something along the lines of...
CHIEF: "She had to tell us about your birthmark Max, she was proving who she was. 99 is the only one who'd know about it."
MAX: "Her and Linquist."
CHIEF: "Linquist? Who's he?"
MAX: "Dr. Linquist, he delivered me."
CHIEF: "Oh, yes, of course, he would have to know."

In the "The Fugitive" parody of season three, entitled "Don't Look Back", Max is being cross examined by the attorney of the other side. When the man asks Max what his job is, he replies that he's a greeting card salesman (his usual cover occupation) and when asked how much he makes a year, he answers "$35,000 a year." When the man answers that that's silly because that's more than Judge Roberts makes a year, as he makes only $25,000 a year, Max looks confused and says, "He sells greeting cards too?"

Answer: Aunt Bertha and Uncle Abner

Aunt Bertha and Uncle Abner are only seen once, and are the only family of Max's that we see, though many are mentioned. They are reluctant to admit they're related to Max.
Aunt Bertha: "My sister's son! I thought he was your sister's son!"
Uncle Abner: "I don't think so; I hope not."
6. Who is Max's old Army buddy from the Korean War?

Answer: Sid Krimm

Don Rickles, in the two part episode of season three entitled "The Little Black Book", plays Sid Krimm, Max's Army buddy. The episode was not written as a two parter, but Don Adams and Don Rickles were best friends in real life and their comic bantering sent the shoot over time and over budget. Luckily their ad-libs were wonderful material for another show, so they made a part two.
7. Who is Max's idol?

Answer: Herb Gaffer, the legendary Agent 4

If any poor, lost soul said Nancy Drew I don't think there's much hope. Herb Gaffer is in the season one episode "Dear Diary", in the retired spies home. He and Max are alike, knocking things over, spraying ink out of pens, jabbing people with them and saying, "Sorry about that!" afterwards, not to mention sharing the "Would you believe..." gag. Like Mr. Gaffer, Max keeps a diary of all his exploits.
8. What did Max do before he was a spy?

Answer: Nothing; the only thing he knows is spying

According to the season two episode, "Maxwell Smart, Private Eye", Max was recruited right out of college by CONTROL as a "bonus baby spy." He joined CONTROL to "fight the forces of evil and serve on the side of niceness." Besides, he needed the money.
9. What apartment building/apartment number is Max's?

Answer: Apartment #86 at the Cherry Arms Apartments

We see the door of Max's apartment many a time, and always it is inscribed with a number 86, ironically being the same as his agent number for CONTROL. On one occasion in a season one episode, as Max is leaving his apartment building, we can just barely see a sign reading "Cherry Arms Apartments" above the door or at least near by it. It is located in Washington, DC.
10. What is Max most afraid of?

Answer: Gollywoggles and lightning

In the season three episode "Witness For The Persecution", Max spends two weeks at Dr. Bascom's house. He refuses to turn out the light because the "gollywoggles might get me." After being banished to another room, Max comes running back in, leaps onto Dr. Bascom's bed, and turning on the light, announces, "If there's one thing I'm more afraid of than gollywoggles, it's lightning!" In a later episode, Max and 99 are trying to escape KAOS, who have emptied out an amusement park and are now trying to kill them. 99 suggests hiding in a certain "house of horrors" type fun house, and after glancing at it Max turns back to her and cries, "Are you kidding, there are gollywoggles in there!"
