1. In "Greatness Achieved", the team was one step closer to bring down The Company when Michael, Lincoln, Sucre and Bellick worked underground to gain access to the General's building. However, one of the team members made an ultimate sacrifice. Who died in this episode?

Answer: Bellick

The gang worked underground and they came to a dead end where there was a big water pipe standing in their way. Michael said if they could not go over or go under it, maybe they could go through it. So Michael's plan was to drill two holes at both sides of the water pipes and then inserted another pipe through it. Little did he know that this plan would cost his friend's life. During their final effort and attempt, the beam broke because the pipe they were going to insert though the water pipe was too heavy.

Their time was running out and Bellick made the decision to go into the water pipe to support the pipe to make sure that it went through the water pipe. He trapped himself inside and drowned.
2. In "Greatness Achieved", Mahone tortured Wyatt by shocking him with the AED (Automated External Defibrillator) every time he became unconscious. Mahone demanded him to call who and apologize from him/her?

Answer: Pamela

Wyatt killed both James and Cameroon, Mahone's son. Mahone wanted him to call his wife, Pamela and apologized. Wyatt refused initially but he just could not stand the agony and torture. He finally gave up and made the call. Then, Mahone tied a weight around his hand and pushed him into a river.

Answer: Trishanne mentioned Whistler accidentally.

Trishanne tried to cheer T-Bag up so she chatted with him but as she mentioned the name Whistler, T-Bag was shocked. Trishanne covered up the story by telling T-Bag that she heard the name from Michael. Later, it was revealed that Trishanne was actually a cop who worked for Don Self. T-Bag called the referee listed in her job application form and Don picked up the call.
4. In "The Legend", Mahone went and looked for David, pretending to be a man from The Company. David knew he was not so he did not give Mahone what he wanted. Eventually, David Baker's wife gave Mahone a piece of paper. What was written on that paper?

Answer: The legend of the blueprint.

Gretchen gave Michael the rest of the missing pages from the bird book. Michael found out more clues from those pages but the symbols did not make any sense. He knew something was missing. Then, Mahone and him worked out that the building that housed Scylla was designed by David Baker, who left his name on the blueprint.

The paper listed out the meaning of each symbol that was drawn in the blueprint.

Back in Sona (season 3), T-Bag set the prison on fire. All of the prisoners escaped and ran for their lives. Sucre did the same thing but he tripped over. He thought that he was going to die at that moment but there was a person behind him who pulled him up; it was Bellick. Bellick was sadly missed by all of the gang, including T-Bag. Sucre made the call to inform Bellick's mother about his death.

Sucre later told Michael about these devices. They knew that if there was any breathing human being weighing more than two pounds in the chamber, it would trigger the thermal alarm. Mahone suggested to the group that they brought in liquid nitrogen into the chamber to cool off the surrounding.

The phone call was actually a trap set by Gretchen and T-Bag. T-Bag knew Trishanne was a cop so he alarmed Gretchen about this. Later, Gretchen asked help from Feng. She told Feng that she faced some problems in retrieving the Scylla card so Feng agreed.
8. In "Selfless", Michael finally got his hand on the Scylla. In order for him and the rest of the gang to leave the building, Sara held Lisa at gunpoint and asked her to make a call to Pad Man. What was revealed in their conversation? That Lisa was Pad Man's ___.

Michael held Pad Man in his own office. Pad Man said Michael and his gang had no place to run anymore. Then, Lisa called and asked his father to let Michael go. It turned out that Michael's original plan was to held Lisa Tabak as the leverage and Sara was assigned to do this.

After killing Feng, Trishanne went back to her company to save her colleagues. She captured T-Bag. However, Gretchen managed to run away.

When Michael's ambulance did not show up, he called Self, only to find out that the number was not available anymore. He knew something was going wrong so he opened the envelope. To his biggest surprise, there were only blank papers inside it.
