This is mentioned during the ceremony on the Cairo Space Station. However, Johnson is sometimes just called "sergeant" in some of the cinematics. Johnson was merely a sergeant in "Halo: Combat Evolved", yet he was placed in charge of the Earth forces whenever Captain Keyes was not available. I can only assume that all the commissioned officers were killed.

You need to pay attention during cinematics, or watch the credits, to figure this out. Keyes is the name of the captain in "Halo: Combat Evolved". Veers commanded the assault on Echo Base in "Star Wars: The Empire Strikes Back". Lord Admiral Horatio Nelson was a British naval officer and hero.

Answer: The Grunt Rebellion

The Prophet of Mercy tells this to the Arbiter during a cinematic. Although the Sack of Reach was an event in "Halo: Combat Evolved", no Arbiter was present. The Taming of the Hunters is mentioned in the afore-mentioned cinematic, but the Brutes are not mentioned. The Massacre of the Innocents happened during World War I.

The three prophets' names are mentioned several times in the game. Truth appears to be the leader of the prophets, as Regret and Mercy are constantly bowing to his whims.

The Pillar of Autumn was Captain Keyes' ship. The Truth and Reconciliation was a Covenant ship that the Marines and the Master Chief had to board to rescue the good Captain Keyes. I just made up Jade Flame.

This is mentioned when the Master Chief is being calibrated on the Cairo Space Station. This segment also informs us that the Humans have managed to develop new technologies whilst the Master Chief was on the first Halo; some technical aspects of the Spartan II battle suit, like the energy shield, have been drastically upgraded.

The other names are all related to Hell, as Tartarus was the Ancient Greek word for Hell. This use of dark, evil verbiage is often utilized by Bungie; all Covenant vehicles are named after undead spirits (i.e. "ghosts," "banshees," and "phantoms.")

Also mentioned during a cinematic. These gigantic rings were built by the Forerunners as a last resort to ensure that the Flood would be destroyed; the Rings cause complete destruction on a galactic scale.

Answer: Forerunners

This is mentioned in both "Halo 2" and "Halo: Combat Evolved" several times. The Forerunners were an ancient super race that built the Halos, as well as other installations all over the galaxy. They sacrificed themselves as a means to defeat the Flood.

Answer: Gravemind

You have to watch the credits to learn this. Gravemind is probably the most spiritually significant being in the game. Gravemind is meant to represent Prince Lucifer. The Flood are meant to represent the demons and minions of Hell, who serve him. Also, like Prince Lucifer, Gravemind takes no sides in mundane conflicts; he seeks only to destroy all life.
