This was the first explosion in a turret of what was considered a cold gun in the U.S. Navy since 1895. That was the year the Navy started using large caliber deck guns that were breach type loading and utilized gun powder in bags.

Answer: USS Samuel B. Roberts FFG-58

The mine made a 25 foot hole in the engine room and injured 10 Sailors. Even though USS Samuel B. Roberts was severely damaged, she was saved by the aggressive efforts of her professional crew. Ironically, during World War II another ship bearing the same name, USS Samuel B. Roberts DE-413, was sunk in action at the Battle of Leyte Gulf.

The U.S. Navy Cruiser, USS Vincennes shot down the Iran Air passenger jet over the Strait of Hormuz resulting in the death of all 290 passengers. The Vincennes' crew, not trusting in their ship's weapons system, misidentified the Iranian Airbus as an attacking F-14 Tomcat. An investigation would eventually show that the airliner had recently taken off and was climbing.

The ship Aegis weapons system did not identify the plane as a threat since it was climbing as its flight path tracked over the ship and was not diving towards the ship in an attack run.

The terrorist attack on USS Cole killed 17 sailors and injured 42 more. Interestingly, charges have been dropped by the U.S. government against the Saudi man who was accused of masterminding the bombing.
