Answer: On the road

The story opens with a teenage girl, Angelique, as she runs through the forest. Clearly scared, she keeps looking behind her as she weaves her way through the trees to the road. When she finally sets foot on the pavement, she is nearly hit by an oncoming car driven by two people who are on their way to work at a medical clinic.

They nearly drive into her, but swerve out of the way just in time. Although Angelique claims that she isn't hurt, she allows them to give her the ride. Arriving at the Lincoln County Womens' Health Service, they instruct a few workers to get Angelique a wheelchair and take her into the building.

As they take her inside, she thanks them for finding her when they did and she notices a red van driving up the road to the parking lot.

She tells them not to let the van in and they quickly gate off the entrance.

Answer: Dwayne

The man in the van turns out to be someone that the staff knows, and evidently, he's violating his court order by sitting in his van outside the gate. A doctor comes outside and tells the guard to make him leave. The guard moves up to the window telling 'Dwayne' that he needs to go.

It doesn't take long before everyone discovers that 'Dwayne' is Angelique's father, and soon, Dwayne has the guard ask to bring Angelique out to his van. The people outside are confused by this, and they tell Dwayne that they'll check up on Angelique for him, but he has to be outside 500 yards of the gate. Dwayne says that he'll only give them fifteen minutes.

The doctor and nurse that brought Angelique to the clinic tell her that she needs to be quick so that she can go back to Dwayne and they have her take her sweater off to reveal a pregnant stomach.

She tells the duo that God brought them to her because they need to help her kill her baby.

The doctor and nurse speak with their boss who tells them to do a quick examination on her. They ask for a psych consultant to come in for a quick exam but he denies them this help until they find something genuinely wrong with her. The doctor expresses concern for his own safety (at the hands of Dwayne) before going to the office and checking on Angelique.

They discover that she's only fifteen years old and sexually active before she opens up and tells her story of what happened. She was home alone late and Dwayne had been out at work. Rather than do her homework, she stayed up and listened to records.

She soon reveals that this only happened the previous Saturday. The doctor finds this odd as she is clearly further along in her pregnancy but she reaffirms the fact and says that no matter what she says, they won't believe the truth. All she wants, she says, is the abortion.

The doctor and nurse later tell their boss that they think she was raped earlier and she continues to block it out.

Their boss gives them more time, telling them to talk Dwayne down first, then give her an ultrasound.

Answer: During the ultrasound

The doctor heads outside and talks to Dwayne who makes his way closer to the building. He gets out of his van and tells the doctor that since Angelique is only fifteen, he rightfully should have his daughter with him when he says so. He also accuses the doctor of working for a slaughterhouse.

The doctor says that Dwayne can't come in since they know what a threat he is to their clinic and he reinstates the fact that they will continue testing Angelique until they know what's wrong with her. The doctor returns to the office and he and the nurse prep Angelique for her ultrasound. Once they begin to take the video, the baby moves inside Angelique's stomach, pushing her skin more than normal.

She screams in pain and the doctor is confused as to what is going on.

They try twice more before the 'baby' inside her folds Angelique's skin over and snaps the ultrasound equipment in half with her body. The doctor and nurse quickly realize that they need to do what she says and perform the abortion.

While Angelique heads to the operating room, Dwayne parks the van next to the fence and opens up the back door to let his three sons out. He instructs them to get to work at the situation, telling one to cut the phone lines before they proceed. While this is done, he moves down the road to pray and as he does so he hears an omniscient voice tell him to 'protect the baby'.

He replies that he will. He returns to his son and takes his picks from a cache of weaponry in the back of the van. He tells the boys their mission as he hands them each a pistol. Dwayne slowly drives up to the gate once more and tells the guard to open it.

While he keeps the guard occupied, one of his sons (who doesn't want to harm anyone) holds a gun pointed at him.

When the guard turns around and prepares to get his own gun out, Dwayne takes a shot at the man's head, killing him. The other two brothers use shotguns to take out the lock on the gate while the people inside worry about the gunshots. The head of the clinic tells the receptionist and a few of the patients to hide in the back while he locks up the front doors, gets a gun, and straps on a bulletproof vest.

The doctor and nurse wheel Angelique into the operating room to let the others hide in the office and the doctor realizes that Angelique's stomach has doubled in size during that short time. Realizing that there is no logical explanation, he hears out her story. That night, after listening to records, Angelique went into the backyard and sat down in the swing set looking at the stars.

She didn't recognize any noises for a long while, but after she heard it, it was too late. She heard thunder, not from the sky, but from the ground.

As she noticed this, two hands pulled her into the ground and she was sexually attacked by whatever pulled her under. When it was finished with her, it brought her back to Earth. It was then that she realized she was impregnated by a demon, and that the only way to stop the evil was to come to the clinic.

Dwayne's sons don't find another entrance into the clinic so they take tot he front door. Dwayne's one hesitant son stays outside, knowing that he doesn't have it in him to go in, and Dwayne allows him to do so and stand guard for anyone who tries to escape to the police. The others begin blasting away at the front doors, taking out wood panels, locks, and panes of glass in a shower of gunfire. The head of the clinic comes out and begins to shoot at the boys but misses and retreats back down the hallway. As they follow up to the doorway, Dwayne talks to him about choice, saying that it was their choice to be there, and the doctor's illegal choice to keep his daughter in such an unsafe place. As they advance down the hall, they keep shooting. One of Dwayne's sons is shot in the neck and he dies of blood loss. Dwayne leaves him behind.
In the operating room, the doctor realizes how big Angelique has gotten and says that she now needs to deliver the baby since she's going to go into labour. She refuses, picks up a scalpel, and threatens to stab herself if he doesn't help, but they are able to restrain her and inject her with a sedative before she can inflict this upon herself.
While all this happens, the patients from the waiting room and the other workers fight over whether or not they should leave.

Dwayne and his other son continue on down the hallway and Dwayne tells the head of the clinic that he must've expected trouble for doing something so wrong since he had all the guns. As he turns the corner and fires his shots into the man's bulletproof vest, he's shot in the arm and reels back around. He and his son take two vantage points and shoot at him high and low before he is subdued. They kick the gun away and take off his vest before clearing off the table and placing him on top. Dwayne goes on about human empathy before picking up a knife and making an incision. Using the tools that are used in the clinic, he performs his own abortion, removing the man's insides with a medical vacuum.
In the operating room, the nurse discovers that Angelique is rapidly dilating before she is sprayed with a viscous white goo from under the sheet. She screams in pain as the liquid burns her eyes and the doctor goes to help. Angelique screams from her seat and says that she can feel the baby coming.
The patients in the back room make their way out as the head of the clinic screams from his office only to run into the third son, standing guard with his shotgun. The family quickly attempts to go for the car while the father tries to reason with the boy, saying that no one needs to know and his daughter never got her abortion. The son shoots him anyways, preventing him from leaving.

Angelique gives birth to the 'baby', but it's anything but a child. The demonic creature makes its way out and crawls around the floor on its six legs while the doctor and nurse hide in a closet. Soon, the lights flicker and the power goes out completely, turning on the generator. Angelique realizes that 'he's here.'
The remaining patients and workers watch in the other room as the ground and tiles on the floor crack open and steam pours out of the fissure. As one worker goes to investigate, he is pulled into the hole and the demon father rises in his wake. The remaining humans scream and crawl into the corner of the room in fear.
Dwayne and his son finish with their first mission when the power goes out and they proceed to the operating room to find Angelique sleeping in her seat. Dwayne goes on to ask God not to punish her and says a prayer as the demon father makes its way through the clinic. Dwayne's third son, who waited outside, walks into the building and comes face to face with the demon, which takes its time to kill him. As Dwayne and his remaining son hear this, a small squeal of the demon child diverts their attention.

As Dwayne and his son search for the baby, the son discovers the creature on the floor and backs out of the room, deciding that it was all too much for him. Dwayne finds the child as well and asks God for guidance since he followed everything he was told and it doesn't make sense. Angelique tells her father that the father of the baby has arrived and he goes outside the room to discover the demon waiting for him. Soon, Dwayne realizes that he never heard the voice of his God.

He actually mistook the demon's words for holy guidance. Angelique removes herself from her seat and takes the gun off the table before dropping to the ground and cooing to her baby, which still sits in the corner.

While the demon father kills Dwayne, the demon child recognizes its mother and as it raises its head, Angelique takes a well-aimed shot and kills the creature.

The demon father walks in and finds its child on the floor. Picking it up, he screams in mourning and walks out of the operating room. The doctor and nurse leave their hiding spot to help Angelique, who says that 'God's work is done.'
