The story opens with a couple driving down an empty road at night. Cliff drives the car while his wife, Abbey, talks about the fact that she's quitting smoking, ending with her last cigarette right then and there. Cliff says that he is happy for her, and that he couldn't be more pleased with the fact since he wants her to be around for a long time.

He states that he'll never let her go. Abbey quickly takes off her belt and turns around to fetch something from the back seat, but as Cliff rounds the corner, the car hits a fallen tree, and though Cliff is buckled tightly into his seat, protected by airbags, Abbey goes through the windshield as the car flips over. Cliff struggles, but manages to get himself out of the car after the crash, only to find Abbey in front of the overturned vehicle, and as he calls 911, neither of them notice the gasoline dripping onto her. Soon, the car sets on fire, and Abbey is set ablaze with it.

Cliff wakes up in the hospital, bloodied and bruised, and he is told that although he is alright, his wife is in critical condition. He is led to her room to find her attached to numerous life support machines, and wrapped entirely in bandages. The doctor tells him that although it may seem like she is responsive, it is merely a reflex, and that she can't hear or see because she is unconscious.

As well, due to her nerve damage, she can not feel anything. Cliff and the doctor speak in the hallway about the possibilities. Regardless, Abbey needs a full body transplant as long as they can find a donor within the next few days.

As well, after the fact, there is the possibility that she will learn to communicate again, but she will never be the same. Cliff notes that Abbey loved to live life, and none of these are really 'living', so to speak. Cliff speaks with his attorney, who tells him that he'd be doing the right thing by ending her life because he knows their values.

As long as Cliff doesn't feel guilty, then it's okay. Cliff says he does feel guilty though, since he cheated on her before the crash.

Cliff returns home alone and leaves his bag at the door. When he goes to the bedroom, he finds two wine glasses on the counter next to a music box, and he decides to open it and reminisce. When he does this, one of the glasses falls off the table and breaks. Shortly after, he hears a splash outside and he looks towards the pool only to find that no one is there.
That night, Cliff makes his dinner, setting a pot on the stove and leaving it to boil while he sits in the other room. Along the way, he spots a note left for him on a whiteboard saying 'Wake me at 7', from Abbey. During this time, he remembers getting ready for a party, and having his wife find his cell phone, on which he filmed his sexual infidelity with his secretary, Trish. He tells Abbey that it's over with her and she drops the phone. As she does this, his encounter with Trish changes into the image of Abbey burning in front of the car. Cliff awakens to find his meal has burnt on the stove, and when he looks up, steam has erased his whiteboard leaving only 'Wake me' written.
That night, Abbey flatlines in the hospital.

Before Cliff goes to the bed, he runs a warm bath and decides to relax. While he does this, the stereo turns itself on, playing "Ave Maria", and causing Cliff to cry. While he shuts his eyes, a footprint evaporates the water around the bathtub and enters it with him. Cliff opens his eyes to find Abbey with him in the tub and he smiles lovingly at him. While they proceed to make love in the water, Cliff realizes that she is soon changing, morphing and rotting away. He throws her off of him and she disappears.
In the hospital, the doctors use the defibrillator to jolt Abbey awake again.
Cliff calls Ira after the bath and lets him know about what happened. When he got out of the bath, he discovered that he was burnt in all the places that Abbey touched him. Ira just says that it's all the stress, and that he doesn't need to worry. Cliff says that when Abbey gets mad she's relentless, and though perhaps she hasn't left the hospital physically, her soul may have come to the house. When Cliff gets off the phone, he hears a knocking at the door, and he opens it to find Abbey's mother, who is shocked that Cliff was going to pull the plug.

The following morning, Cliff turns on the TV to find that Abbey's mother is on TV, calling Cliff abusive and saying that it should be God's choice as to whether Abbey lives or dies. Cliff leaves and heads to work, and when he arrives, he discovers that he has 34 new messages, each one a varying viewpoint as to whether or not he would do the right thing about letting Abbey live or die. Trish appears soon after, asking him where he's been.

He mentions he's been in a hospital and his wife may be dying, but she is more concerned with herself, saying that she was fired by Abbey, and is now a month late on condo payments.

She tells him that she wants to be on fire, and he says that he doesn't want to sleep with her anymore, so he leaves. He decides to visit Abbey in the hospital, but when he arrives to drop off flowers, he finds that doctors are washing down her body, removing rather large portions of charred skin.

As Abbey turns to look at him, be decides to leave.

Ira tells Cliff that Pam is set to appear on the 'Montel Show', and Cliff is surprised that the situation is blowing so hugely out of proportion. He asks Ira if he knew that this would happen when they were in the hospital and Ira says he didn't, but Ira is trying to generate press in his favour.
At work that day, Cliff tries to perform oral surgery, but Trish tries to seduce him in the office. He gets a call from Ira and is told that a 'do not resuscitate' order has been placed into effect by the judge. He also tells Cliff to bring lots more flowers, as the press will be watching.
Ira decides to visit Abbey's room, despite the fact that she never liked him, to pay his farewells to her. He ends up saying that although she was in a horrific accident, he had to take advantage, because when life gives you lemons, you make lemonade. He toasts her and lemonade only to see her turn her head to face him. He leaves shortly after.

Cliff arrives at the hospital once more as Ira is about to leave the room and he asks for some time alone with Abbey. When Ira departs, he moves over to Abbey's bed, and despite her charred skin, he decides to kiss her on the lips, inadvertently causing her to flatline for a second time.

The doctors usher him out, saying that he can't be here the second time. Cliff is shocked that she flatlined a first time, but even more so when he discovers that it happened when he had the vision of her in the bathtub.

As he walks down the hallway, and while the doctors try to revive her, he has another vision of Abbey. Ira meanwhile, is walking through the hotel with his cell phone, and despite the fact that a doctor asks him to keep it off in the hospital, he disobeys and accidentally enters a room containing an MRI scanner. Abbey's spirit, from behind the control panel, turns on the magnet, causing certain items on Ira's body to pin him onto the door. Slowly, he watches as Abbey's charred body crawls behind him, and he frees himself by tearing his ring free from his finger, removing the whiskey bottle from his jacket pocket, and tearing the watch off his arm.

He isn't able to escape though, and Abbey sets him on fire. Cliff finds his body on fire shortly after, but is unable to save him, and he rushes back to Abbey's room to find that she is once again stable.

Realizing Abbey's power, Cliff tries everything in his power to keep her alive; otherwise he may be the next victim. He races outside to where Pam is speaking with the press and he retracts his statement, saying that he will personally reward anyone who finds a donor for Abbey to keep her alive.
When Cliff gets home that night, he finds the lights on and candles lit, and he discovers that Trish let herself in, and she has a bottle of wine waiting for him. He tells her to leave since he needs time to think, because he needs a donor for Abbey within a short amount of time or he'll get back at him in her own way. She doesn't leave though. Instead, she strips down, saying that he's been devoid physically and needs to relax. He initially tries to force her out, but gives in to temptation.
When Cliff goes to bed, he can't help but watch the clock. Trish says that when Abbey kicks the bucket, nothing'll happen, but Cliff says that she'll be dangerous. Trish goes to get another bottle of wine.

Cliff stays and bed and remembers the night they crashed. He tells Abbey that they made it through the weekend, so they should head on a drive to the cabin like old time's sake. Abbey agrees. While Cliff daydreams, Abbey's picture falls off of the nightstand and shatters. While Trish is in the other room getting more wine, no one notices the temperature of the house steadily rise. Trish gets the idea to send topless pictures to Cliff over the phone, and as he watches through the slideshow, he sees as Abbey's charred body slowly crawls across the countertop towards Trish. Cliff runs to the kitchen to save Trish, and when he arrives, he looks at the whiteboard on the wall. It reads, in charred letters, 'SKIN'.
As Trish embraces Cliff, he states that since she isn't around, they must have revived her at the hospital. Trish agrees that she needs to live, and that they need to stop her before she kills them both. Cliff tells Trish to get her things, but when she does, he takes a wine bottle from the shelf and knocks her out.
When Trish wakes up, she's at the dentist's office, and she's tied onto the chair. Cliff says that he's sorry, but he needs her to be awake during her surgery, since the skin she has needs to be 'fresh' to be used on Abby. He slowly takes his time to cut along the lines and peel away the layers of Trish's skin and place the slabs into a cooler before thanking her and slicing her neck with a buzzsaw. With only an hour to spare, Cliff drives back to the hospital.

While driving, Cliff remembers that fateful night, saying that he wouldn't let Abbey go, despite the fact that he cheated on her, and that he really did love her. When she unbuckles her belt and turns to get her surprise, she reveals that it's a pregnancy test, and that she's pregnant with his child. This is why she decided to quit smoking. Cliff is excited, saying that they can finally start a family, but Abbey says that she won't let him near her baby, and that he'll be moving out of their house, never to speak to each other again. The car crashes shortly after, and Cliff piles out to find Abbey in the front. When she asks him to call 911, he picks up her phone and punches in the number, but thinking about it, he decides not to dial. Instead, he takes a lighter, and despite the fact that she says that she was sorry, he lights the blaze that burns her.
Coming out of his flashback, Cliff nearly hits another tree turning a corner, and swerving out of the way, he accidentally knocks Trish and all of her severed limbs onto the road. He stops the car and decides to pick them up, but in doing so, he nearly gets caught by the cops, who drive by and neglect to see the severed arm he is holding. Cliff tosses them into the bag and kicks her feet off into the ditch.
With fifteen minutes to spare, Cliff rushes to the hospital with his blood-soaked cooler in hand, and as he creates a leaky trail of blood, he finally reaches Abbey's room to find that she passed away in the night. The doctors suggest grief counseling and say that they have papers for him to sign, but he departs without a sound.
When he gets home, Cliff tosses Trish's corpse and the cooler into a garbage can and wheels it to the curb. When he reaches the front door, it opens, and Abbey moves to the side to let him in. Cliff wipes his feet, then proceeds, and the door shuts behind them.
