Answer: Snivy

Snivy, nicknamed the "grass snake" Pokémon, is the starter Grass Pokémon you can choose at the outset of the game. It looks like a green bird with yellow underparts, and brown snake-like eyes. The best stat is speed: it can move faster than the other starter Pokémon (Tepig for fire and Oshawott as the water starter).

Snivy's speed is enhanced by sunlight. When Snivy is low on sunlight energy, its tail will droop.
As a Grass type Pokémon, Snivy will have an advantage when attacking Water type Pokémon such as Oshawott, against Ground type Pokémon (for instance Drilbur) and against Rock type Pokémon such as Tirtouga.

Contrary to most other generations, the grass starter Snivy is not the first Pokémon in the local Pokédex - that is the mythical Victini, that one can only find on special occasions.
And although a starter, Snivy is not the weakest Pokémon in Generation V. There are several Pokémon with lower stats, and Patrat is the weakest of them all.

At level 17 Snivy can evolve to Servine, a more slender bird. And at level 36 it can evolve to Serperior.
2. Tungsten - Astatine - Carbon - Hydrogen - Oganesson

Answer: Watchog

A Watchog is the evolution (at level 20) of a Patrat. Watchog is nicknamed the Lookout Pokémon and will raise its tail if it discovers other Pokémon nearby.
Watchog is a Normal type Pokémon. As such, fighting a Ghost type Pokémon such as a Yamask will have no effect whatsoever.

The physical appearance of a Watchog may be based on the American groundhog, but to me it resembles more an African meerkat: it is more slender than a groundhog.
When you start your journey through the world of Unova, you can find Patrat or the evolved Watchog already on route 1, but also in some other places.
3. Yttrium - Americium - Arsenic - Potassium

Answer: Yamask

A Yamask (nicknamed the Spirit Pokémon) is a Ghost type Pokémon, a very good attacker against Psychic type Pokémon such as the Gothita.

The artwork for Yamask resembles not any existing animal. The best description would be a black bird head with large black wings, and carrying an egg-like mask in its black tail.

The Yamask can evolve into a Cofagrigus (resembling a sarcophagus with four hands) at level 34.
You can only find Yamask in the wild near Relic Castle.
4. Argon - Carbon - Helium - Nitrogen

Answer: Archen

Archen is a so-called Fossil Pokémon: you can obtain a fossil in a certain place and bring it to life in another place. In the case of an Archen, you can obtain the fossil either in the Relic Castle or in the Desert Resort, and revive it in the Nacrene City Museum.
Archen is nicknamed the First Bird. It looks a bit like a parrot, but with a stony beak. The colours are usually red for the back, yellow for the chest and blue at the tip of the wings and the tail, but the shiny version replaces the blue with bright green.

Archen is a quite good attacker. On the other hand its defences are rather weak.
Archen is a Rock/Flying type Pokémon. As such, it has a huge advantage when attacking a bug type Pokémon such as the Karrablast.
Archen can evolve at level 37 into an Archeops, which looks to me like a bird of paradise (sporting the same colours as the Archen, though).
5. Rhenium - Uranium - Nickel - Carbon - Lutetium - Sulphur

Answer: Reuniclus

The Reuniclus is an evolution of two previous Psychic type Pokémon. The basic form is the Solosis, which can evolve into a Duosion, and in term the Duosion can evolve into a Reuniclus.
The Reuniclus is nicknamed the Multiplying Pokémon - not because it tends to multiply physically, but it multiplies the strength of its psychic power when it shakes hands, and then it can crush large rocks.

Reuniclus is a good attacker, albeit quite slow. It has an advantage when attacking Fighting type Pokémon such as Throh.
The artwork for a Reuniclus is very odd. It vaguely resembles a bunny with extremely large front paws ending in fingered hands, but all of its body is outlined with some greenish liquid. Being a Psychic type Pokémon, it is quite evident that a Reuniclus' appearance does not match with out-of-game reality.

In "Pokémon White" and "Pokémon White 2", you can encounter Reuniclus on route 9. In "Pokémon Black" and "Pokémon Black 2" however, the species does only appear when trading with another game. 
6. Einsteinium - Calcium - Vanadium - Aluminium - Iodine - Erbium

Answer: Escavalier

The Escavalier is nicknamed the Cavalry Pokémon. However, it is quite slow. It has a very powerful attack and a good defence.
You can evolve an Escavalier out of a Karrablast, but only by trading with Shelmet - not by levelling up.

How to describe an Escavalier? Well: start with a snail. Add a wasp as chest and top with a Greek or Roman helmet. On both front paws it carries a small round shield and a long lance. In the normal appearance the crest of the helmet is red or very dark pink, while the shiny version has a lighter hue. The lances are striped in the same colour as the helmet's crest.

The Escavalier is a Bug/Steel type. As such, it has a little advantage when attacking a Grass type Pokémon (for instance Snivy) or an Ice type (for example the Vanillite).
7. Aluminium - Oxygen - Molybdenum - Molybdenum - Lanthanum

Answer: Alomomola

The Alomomola is a Water type Pokémon, nicknamed the Caring Pokémon. This nickname comes from the Alomomola's habit to embrace wounded Pokémon and thus heal them.
It resembles a pink fish, with eyebrows that reminds me of young women.

One of the better Pokémon as to Hit Points, it lacks however in special attack and special defence.
As a Water type, it has an edge when attacking Fire type Pokémon such as Tepig.
8. Potassium - Lithium - Nitrogen - Potassium

Answer: Klink

Klink is a Steel type Pokémon. Its nickname is the Gear Pokémon, based upon its physical appearance: it looks like two interlocking cog wheels.
You can find some wild Klink in the Chargestone Cave.

Klink can evolve at level 38 into Klang, which in turn can evolve further to Klinklang.
Klink is a better defender than attacker. When attacking, it has an edge over Rock type Pokémon such as the Roggenrola.
9. Chromium - Yttrium - Oganesson - Oxygen - Nitrogen - Aluminium

Answer: Cryogonal

Cryogonal (also known as the Crystallizing Pokémon) is an Ice type Pokémon, and that is clearly reflected in its artwork: an ice crystal in shades of blue and grey.
The Cryogonal can almost exclusively survive in the winter season. It forms ice crystal chains to capture prey and freeze it to death.

As all Ice type Pokémon, Cryogonal has an advantage when attacking Flying type Pokémon such as the legendary Tornadus.
You can encounter some wild Cryogonal near Twist Mountain. Be careful: they're very hard to catch.
10. Rhenium - Sulphur - Hydrogen - Iridium - Americium

Answer: Reshiram

Reshiram is a Dragon/Fire type Pokémon, one of the legendary Pokémon from Generation V.
Its artwork looks like a pure white chicken, with a jet engine as tail. And as if that isn't weird enough, it sports very long hair on its head - flowing down towards the jet engine.

Reshiram is a fiendish Pokémon: the base friendship is zero indeed.
As a legendary Pokémon, Reshiram has good stats all over. But its best stat is the special attack: when it ignites its tail, the surroundings are scorched - which also can influence the overall weather circumstances.

Not all Generation V games have Reshiram. "Pokémon White" and "Pokémon Black 2" have Zekrom instead, a legendary Dragon/Electric type. In "Pokémon Black" and "Pokémon White 2" you can find Reshiram near Dragonspiral Tower.
