Answer: Loki

Loki is Thor's stepbrother who tried to get Thor to fight the Hulk. The message was intercepted by Ant Man, Wasp, and Iron Man. When the plot against Thor was revealed the Avengers were formed by Ant-Man, the Wasp, Thor, Iron Man, and the Hulk (who didn't remain with them long).
2. In issue number 4 of their original series, the "Avengers" discovered this 'Golden Age' hero floating in the sea, having been frozen in ice for decades. Who is he or she?

Answer: Captain America

Captain America had been frozen in ice. This block of ice was thrown by Namor, the Sub-Mariner into the sea, where it drifted until Captain America was partially freed from the ice when the "Avengers" submarine picked him up.
3. Most of the original line-up of the "Avengers" left leaving Captain America in charge. Who replaced them?

Answer: Hawkeye, Scarlet Witch and Quicksilver

All of these people have at one time or another been members of the "Avengers", but the first line up change, after the Hulk's departure and Captain America's joining was the recruitment of Hawkeye, Scarlet Witch and Quicksilver.

Spider-Man did indeed turn down an offer of membership. Nova (Richard Rider) would love to be a member, Ms. Marvel has been a member. Thanos is a stellar class villain. However in Issue #3, Spider-Man accepted the offer and became an Avenger.
