Although it bears the cover date of Jun. 1938, "Action Comics" #1, Vol. 1 was actually released on April 18, 1938.

The original Ultra-Humanite was introduced in "Action Comics" #13 and met his end in "Action Comics" #21... by taking a swan dive into a volcano.
