He was looking for his wife, whom he thought was hiding in the crawl space. After he went in to look for her (and didn't find her) the door closed behind him. He became frightened and frantically pounded on the door for his wife. He heard something behind him and as he turned around he found himself face-to-face with Mary Shaw. Deathly afraid, he screamed loudly and met his untimely end.

Yes, it was Ella - his father's new and much younger wife. It was implied that she had taken over Mary Shaw's practice of creating the "perfect doll". She was played by Amber Valletta. Amber Valletta is also known for her role as Allegra Cole in the movie "Hitch".

As it turned out, Jamie's great-great-grandfather was the boy who mocked Mary Shaw during her performance. It was he whose death Mary Shaw was thought to be responsible for. Before she died, she vowed revenge on the families of those who killed her. Jamie found out that Mary Shaw killed Lisa because she was pregnant with Jamie's unborn son (thus the last person who would carry on the Ashen family name). Unfortunately for Jamie, he met his end as the movie ended.
