1. What is the name of the creature on Hoth that attacked Luke and killed his Tauntaun?

Answer: Wampa

It ties Luke upside down in a cave. When Luke wakes up he uses the force to retrieve his light saber, and chops off the Wampa's arm. However, he would have perished in the freezing Hoth night if Han Solo did not rescue him.
2. What planet does Luke visit to find Jedi Master Yoda?

Answer: Dagobah

Right before Luke passes out from the extremely cold temperatures on the Hoth system, he sees Obi Wan Kenobi, who tells him to go to Dagobah and learn from Yoda. He then passes out and is not sure if this occurred when he wakes up.

Answer: Admiral Piett

He was promoted after Admiral Ozzel took the Imperial Fleet out of light speed too quickly in the attack of the Hoth System. It alerted the rebel base and they put up their deflector shield. Admiral Ozzel was choked to death by Vader. Admiral Piett loses the Millennium Falcon in the end, but Vader lets him live.

Boba Fett is actually a clone of Jango Fett who followed in his father's footsteps and became a bounty hunter. He owns the "Slave I", which was his father's ship. He takes Captain Solo to Jabba the Hutt once Han is frozen in carbonite.

Answer: Cloud City

The cloud city of Bespin was another refuge for people who did not want to be found and was run by Lando Calrissian. However, once Boba Fett traces the Millennium Falcon there, Darth Vader makes Lando turn over Solo, Chewbacca, and Leia (his daughter although unbeknownst to him) to him and states that he might have to end up leaving a garrison there.
6. What does unheralded hero R2D2 do to save the day in "The Empire Strikes Back"?

Answer: He fixes the hyper drive on the "Millennium Falcon".

He, of course, does all of these things, but in different movies. He realizes that the Empire deactivated the hyper drive on the Falcon and ignores C3PO's complaining about his detached legs and fixes it and lets the crew get away.
He also fixes a hyper drive in "Phantom Menace", saves Padme from molten liquid in "Attack of the Clones", saves them from a garbage chute crushing them in "A New Hope", and gets doors open on the forest moon of Endor in "Return of the Jedi".

Towards the end of the film, after Luke is defeated by Darth Vader (who we now know is his father), he is left hanging from the garbage chute over the Cloud City of Bespin. Obi-Wan had stated that he would not be able to help him so Luke called for his sister, Leia. She had heard his words and then had Lando turn the "Millennium Falcon" around.

Answer: 2-1B

2-1B also helps Luke out near the beginning of the film when he almost freezes to death on Hoth. R4 was the Astro droid with Obi-Wan Kenobi in "Attack of the Clones". R5-D4 was the unit that malfunctioned in "A New Hope" which prompted Owen Lars to buy R2-D2, asking a Jawa, "How about that blue one?"
IG-88 was a bounty hunter who was hired by Vader to find the Millennium Falcon. Boba Fett had found the ship first, however.
