Capone had headquarters in Chicago proper in the Four Deuces at 2222 S. Wabash, the Metropole Hotel at 2300 S. Michigan Avenue, and the Lexington Hotel at 2135 S. Michigan Avenue.

He expanded into the suburbs, sometimes using terror as in Forest View, which became known as 'Caponeville.' Sometimes he simply bribed public officials and the police as in Cicero.

He established suburban headquarters in Cicero's Anton Hotel at 4835 W. 22nd Street and in the Hawthorne Hotel at 4823 22nd Street. He pretended to be an antique dealer and a doctor to front his headquarters.

Known for his flagrant romancing and promiscuity, it is believed he was treated for the disease in 1927, but never cured. He was in a partially vegetative, demented state during the last 18 months of his life, dying on Jan 25, 1947 at the age of 48 at his Miami home.

There are those who question syphilis as the cause of death. Nevertheless, on November 16, 1939, Al Capone was released from Alcatraz after having served seven years, six months and fifteen days, having paid all fines and back taxes. Suffering from paresis derived from syphilis, he had deteriorated greatly during his confinement.

The infection spread to his brain and he was a virtual vegetable most of the time. Immediately upon release he entered a Baltimore hospital for brain treatment, and then went on to his Florida home, an estate on Palm Island in Biscayne Bay near Miami, which he had purchased in 1928. Nevertheless, Capone had numerous system failures due to being in the third stage of the veneral disease.

Therefore, the Dade County Medical Examiner attributed his death to 'numerous complications including renal, hepatic, and congestive heart failure due to end stage syphilitic complications'. So, from this, one can infer the true causation of his death was directly attributable to the last fatal stages of syphilis (Source FBI website keyword search 'Capone Autopsy Report' and 'Capone').
