Locke was still asleep when the episode began with a flash sideways scene but then he began to slowly open his eyes. Right as he pointed the gun at the man, Charles shot his gun and told Sawyer to put his down or else he would shoot Kate dead. Sawyer agreed and relinquished the gun.

During the opening credits, Jack was seen looking for Dr. Nadler who worked at a different hospital than he did. He entered a room and asked a gentleman if he knew where he could locate Dr. Nadler so the man said he was indeed Dr. Nadler but Jack could call him Bernard. Jack asked if he could look at the file Dr. Nadler had on John Locke since Dr. Nadler previously did emergency oral surgery on him. Dr. Nadler said he could not give Jack that information since it would break doctor-patient confidentiality but he informed him there was another man involved in Locke's accident.

Back on the island, Jack was just waking up after he had been injured by an attack in the previous episode and Sayid was right by his side. He asked Sayid what happened so Sayid informed him there was a mortar attack since Widmore and his people fired on them. Sayid mentioned to Jack that Locke saved him after he had been injured from an explosion.

Near the beginning of the episode while on the island, Sun and Jin were speaking and he mentioned to her that Charles Widmore found her camera on the plane. While Sawyer and many others were imprisoned in cages by Charles Widmore and his people, one of Charles' men commented that the power had cut out. They decided to look into the problem but then the Smoke Monster appeared (Smoke Monster is the "Man in Black" but in smoke form). The Smoke Monster killed just Charles Widmore's people.

When Jack and a bunch of others, including Jin and Sun, were on a submarine to try and leave the island, a bomb was on the sub and it exploded. A bunch of water flooded into the submarine and Sun was trapped. Jin went to go help her but she begged him to let her be. Jin told Sun he would not leave her to die all alone so both of them drowned together as water continued to flood inside the submarine.
