Sigmund Freud is a famous 19th century psychologist. William Reich was his student, who later developed the idea of the AT field. The field itself is the theory that a child who grew up without strong parental figures would create a mental barrier, or the Absolute Terror field, to keep others at an emotional distance.

Shinji sits on the sephiroth Tiphereth in the center of the tree, where God begins to manifest and the ritual of crucifiction begins.

The doll is first used in Asuka's past, where her mother commited suicide with a doll, thinking that it was Asuka. This leads directly to the second use of the doll.
