Answer: Dead, in the trunk of a car

ADA Borgia was encouraging a man who was caught making fake police badges to give her the names of his criminal customers. Those criminals burst into her apartment and the last we heard from her, she was screaming just before they taped her mouth shut. She was the only "Law & Order" ADA to be killed in office.
2. What happened to Detective Falco, played by Michael Imperioli?

Answer: Was only temporary while Ed Green was out

Detective Nick Falco, played by Michael Imperioli, was a temporary replacement when Detective Ed Green was recuperating from a gunshot wound in season 15 (and the actor was making the movie version of "Rent"). Falco was partnered with Detective Fontana.

Sharp viewers will remember Imperioli from when he played a coke-head limo driver in "L&O" season 6.
3. ADA Serena Southerlyn, the only blond ADA in the show's original run (except for possibly Michael Cutter), left abruptly in the middle of season 15. What happened?

Answer: DA Branch fired her

Executive producer and series creator Dick Wolf wrote Serena's final scene in a deliberate attempt to make a "water-cooler" moment.

In her final episode, DA Arthur Branch told Serena she was more passionate about the defendants than she should be as a prosecutor and fired her. She asked him if her firing has anything to do with her "being a lesbian" and Branch says, "No", to which she answers, "Good, good", and that was that for ADA Serena Southerlyn.
4. Captain Don Cragen ran the 27th precinct for the show's first three seasons. Where did he go?

Answer: Joined Anti-Corruption and eventually led the Special Victims Unit

At the beginning of season 4, Lt. Anita Van Buren took over running the homicide unit and it was mentioned that Captain Cragen joined the Anti-Corruption task force. The irrepressible character returned in 1998 in the TV movie "Exiled" to help Detective Logan solve a case about dirty cops and then again in 1999 to co-lead the Special Victims Unit on the "Law & Order: SVU" spin-off.
5. Milena Govich played Nina Cassady, partnered with Detective Green, for just one season as the series' only female detective. What happened to her?

Answer: No explanation given

Poor Cassady. She was in way over her head, having been granted the promotion to detective after being a hero in a beauty salon police incident. She hadn't even been to detective school when she was put in Lt. Van Buren's squad, and Van Buren was not happy about it. It didn't help that Nina botched several interrogations.

No reason was given for Nina not returning for season 17, but I think it's fair to assume she was probably fired or demoted.
6. Rey Curtis, played by Benjamin Bratt, replaced Mike Logan in season 6 and was partnered with Lennie Briscoe. Rey left at the end of season 9, because:

Answer: His wife had MS and Rey wanted to take care of her

After cheating on his wife at the end of season 6, the devoutly-Catholic Rey believed his infidelity was what caused his wife's multiple sclerosis when it appeared in later seasons. By the end of season 9, Deborah needed help even brushing her teeth, so Rey left the homicide unit to take care of her.

Ten years later, in the show's final season, Rey returned to New York for Deborah's funeral and he visited with Van Buren. In an interview on NBC.com, actor Anthony Anderson, who was playing one of the detectives in that final season, said he hoped Benjamin Bratt wasn't returning because "he's a lot better looking than I am."
7. When did Jack McCoy leave the show?

Answer: He never left in the first 20 seasons

After DA Arthur Branch left (and the actor Fred Thompson went to campaign for President), Executive ADA Jack McCoy was appointed the interim DA. Eventually there was an election for the permanent position and Jack won. In one of my favorite scenes, after squabbling with new Executive ADA Mike Cutter, McCoy said, "Now I understand why Adam Schiff was so cranky."
8. Connie Rubirosa picked up where ADA Alexandra Borgia left off. What happened to Connie?

Answer: She was there to the end of the first 20 seasons

Connie Rubirosa, played by Alana de la Garza, was the assistant district attorney first to Jack McCoy and then to Mike Cutter. She had a knack for getting more information and cooperation from witnesses than her male associates had. She never left "Law & Order" and was part of its final show, "Rubber Room".

In the Law and Order universe, Rubirosa moved to Los Angeles in "Law & Order: LA" and joined the prosecution there, in order to take care of her mother, who lives in the area. It was probably an attempt at a ratings boost. Sadly, that didn't work, as NBC cancelled the last "Law & Order: LA" spin-off after its second season.
9. Abbie Carmichael joined Jack McCoy's team at the beginning of season 9. She was a tough prosecutor and eventually left Jack's employ in order to do what?

Answer: join the U.S. Attorney's office

We learned about Abbie's style early - when having to work through dinner for the first time, Jack brought her a salad, which she eschewed in favor of his ribs. The tough Texan eventually joined the U.S. Attorney's office.
10. Anita Van Buren was diagnosed with cervical cancer in the beginning of season 20. What happened to the beloved "Lieu" ?

Answer: She got engaged and received good news about her health in the final episode

Lt. Van Buren was a stabilizing factor in the precinct, reliable for pointing her detectives in the right direction and never losing her cool. The good test results in the final episode and the party her department threw for her to raise money for her medical expenses put a positive spin on the season's final episode.

If you want to see S. Eptha Merkerson's acting ability, look for "Mushrooms", the seventeenth episode of the series' first season. She played a grieving mother in a scene you aren't likely to forget.
