Indeed not. Edward Winter showed up first in the 'M*A*S*H' episode 'Deal Me Out', where he played a certain Captain Halloran, also part of military intelligence. It could very well be that this Captain Halloran was just another of Colonel Flagg's aliases.
2. Colonel Flagg has broken his arm, ostensibly why he has arrived at the 4077th. How did this painful mishap take place?

Answer: Helicopter crash

Specifically, Colonel Flagg fractured his radius in the crash, while the chopper pilot miraculously walked away unscathed. Later, it is suggested that Flagg may have fractured the arm himself, something an incredulous Hawkeye has a hard time believing, that is, until Flagg re-injures his own arm using the x-ray machine.

Another time, he met a certain Linda Collins, a 'rent-a-car girl' in Houston during an AMA convention. 'You can count on my full and fearful cooperation,' Henry suggests.

Amusingly, when Colonel Flagg leaves Henry's office, he takes a look at the sleeping Radar, who had just been peeking through the door. Radar's glasses are still on. Quickly, Radar takes his glasses off when Flagg's flashlight leaves his face. Suddenly, Flagg takes another look and sees Radar's face, sans glasses. He doesn't notice. Some intelligence officer!

This is the only time that Colonel Flagg's first name is mentioned, and there is no way to be certain if this is his 'real' given name, so we'll just have to assume it. Happily, he would show up in five more future 'M*A*S*H' episodes!
