Dr. Cox usually has a new girl's name for J.D. each episode. As this episode is an homage to "The Wizard of Oz", it is only right that J.D. is named after its primary character.

At the beginning of the episode, J.D. is relaxing in his bathtub and listening to Toto's Africa" on his iPad. When he has to go in to work, Toto, like Dorothy's dog, travels with him.

Turk is hugging the heart donor's father (having found his heart), Elliot is giving a lecture (having found her brains), Carla is playing with Jack (having found her courage) and J.D has finally found his way home.

This is one of the more famous adaptations of the song, thanks to its peculiar musical arrangement. Big Iz', the artist's, full name is Israel Kamakawiwo'ole. He died at age 38 in 1997 from complications due to obesity.
