Answer: The Jupiter Station

The Doctor was created at the Jupiter station by Dr. Lewis Zimmerman. We find this out when the Doctor's program malfunctions and his memory circuits start to degrade in the episode "Projections, and when he is sent back to help Dr. Zimmerman in the episode "Life Line".

Answer: 5

Voyager was first destroyed in the episode "Deadlock, when there were two Voyagers and one of them was boarded by an alien species, Captain Janeway activated the self destruct sequence and the other Voyager went on its way back to the Alpha Quadrant. Voyager was destroyed a second time was in the episode "Year of Hell: Part 2" when Captain Janeway smashes Voyager into the weapon Annorax was using to destroy civilizations.

Although Voyager was destroyed, history was changed and everything went back to the way it was before the episode started.

The third time was in the episode "Course: Oblivion" when the duplicate of Voyager, created in the episode "Demon", disintegrated from using a trans-warp conduit taken from a Borg cube. Because Voyager was a duplicate, it could not withstand the trans-warp conduit's radiation and they disintegrated, but the real Voyager was nearby.

The fourth time was in the episode "Timeless", when the new slip stream drive was tested, Harry sent the wrong phase variance and caused Voyager to crash land on an ice planet, in the future, Harry sends a phase variance to knock Voyager out of the slip stream and changing history.

The fifth time was in the episode "Relativity" when Voyager dematerializes by a temporal bomb planted by Captain Braxton in the beginning of the episode.

We hear this code used in the episodes, "Deadlock", Concerning Flight", and "Basics: Part 1".
4. In the episode "Waking Moments", how does Captain Janeway try to figure out if the whole thing was a dream?

Answer: She stays in engineering during a warp core overload

While trying to take control of Voyager, Lieutenant Torres accidentally caused the warp core to overload. When Captain Janeway questioned why the warp core wouldn't eject, she realized it was probably a dream. She tested her theory by seeing if she could withstand an antimatter explosion, and she did.
5. Why was Seven of Nine all alone in the episode "One"?

Answer: The crew was in stasis

Voyager was heading through a part of space that was heavily polluted by radiation. Seven was unaffected by it so she was to watch over the crew while they were in the stasis chambers.
6. In the episode "Endgame: Part 2", what was Voyager in when they entered the Alpha Quadrant?

Answer: A Borg sphere

When Voyager was in the sub space tunnels, the Borg queen sent a sphere to go destroy Voyager. The Borg sphere tractored Voyager inside it. Just as it entered the Alpha Quadrant, Voyager fired a torpedo at it and escaped.
7. In the episode "Course: Oblivion", who was the last person to die on the ship?

Answer: Ensign Kim

Toward the end, Seven of Nine and Harry Kim were the last two remaining on Voyager. Harry was trying to contact a distant ship, but was not in range. The ship he was trying to contact, the real Voyager, was also trying to hail him as well. By the time both ships were in range, the duplicate Voyager was destroyed.
8. Captain Janeway has a lucky what?

Answer: Tea cup

We learn in the episode "Year of Hell", when Chakotay goes into Captain Janeway's ready room to help her clean up the mess, that Captain Janeway has a lucky tea cup. Just after this, Voyager is under attack and the tea cup breaks.
9. What speed was said to be impossible to reach?

Answer: Warp 10

In the episode "Threshold", we learn that no one has ever reached warp 10 and that it was a myth. Tom Paris tries to get a shuttle to go warp 10 and succeeds, but not without paying a price. He began to age faster.
10. What deck is the Holodeck located?

Answer: 6

In the episode "Twisted", we learn that the whole ship lead everyone back to deck 7 because it was in the middle of the ship, so everyone took refuge in the Holodeck.
