There are five members of Makarov's terrorist squad during the attack on Zakhaev International Airport in the level "No Russian": Viktor, Lev, Kiril, Allen(under the alias of Alexei Borodin) and Makarov himself. While Lev and Kiril are both killed on the runway, Allen, Viktor and Makarov make it to the getaway vehicle.

However, Makarov is aware of Allen's being of an undercover agent, and kills him and leaves his body for the Russian authorities to discover, leading them to think that the Americans were responsible for the airport massacre.

Although Sat1 was killed during the events of "Of Their Own Accord", it is shown to the player that EMP missile launched by Price exploded above Washington D.C, taking away all power from the state while at the same time destroying the International Space Station. The player takes control of Private James Ramirez in the American Campaign after PFC. Allen is shot and killed during his undercover mission in "No Russian". Ramirez, under Sergeant Foley's command, helps retake the White House after Russians invade and take control of it during the events of "Second Sun" and "Whiskey Hotel", and prevent a U.S. bombing of Washington D.C. by signaling that the U.S. forces have recaptured the White House.

General Shepherd is revealed to be an antagonist near the end of the game, in "Loose Ends", when he kills Ghost with a 44. Magnum gunshot to the head and orders Shadow Company soldiers to douse Roach with gasoline, after which he sets him on fire with a cigarette.

Shepherd directly kills Roach with the cigarette, although the Shadow Company soldiers were the ones who soaked him with gasoline.

During Price's conversation with Makarov over the radio, the player can listen in on what they are saying to each other. Price approaches Makarov with the saying, "The enemy of my enemy is my friend", while Makarov realizes that Shepherd now wants him dead so that he cannot reveal to anyone that Shepherd was working with him. Makarov gives information on Shepherd's whereabouts before saying, "I'll see you in hell, Price." Captain Price responds to this by saying, "Looking forward to it. Give my regards to Zakhaev if you get there first."

Shepherd is killed in the last mission, "Endgame". As Price and Soap chase him through a winding river in Afghanistan, Shepherd's speedboat goes into his Pave low and lifts off of the river. Captain Price tells Soap to slow the boat down, before putting a grenade launcher round into the Armoured helicopter, effectively shooting it down.

However, the boat goes over a waterfall, causing the player-controlled Soap MacTavish to lose sight of Captain Price. As he ventures forward, he finds Shepherd leaning against a car, exhausted and out of breath.

After Soap attempts to stab and kill him, he dodges the knife, rams Soap's head into the car and stabs Soap with is own knife. Captain Price tackles Shepherd, preventing him from shooting Soap. While the two emerge in a fistfight, Soap pulls the knife out of his body and throws it at Shepherd, striking him in the right eye.
