Answer: A skill point from Akara

The first quest in the game is from Akara, who asks that the Den of Evil be cleansed. It's straightforward: go to the Den of Evil, and kill every monster in it (the lighting even changes when it has been completed). Talking to Akara afterwards will net you an extra skill point.
2. (Act I) Where is Super Unique enemy Bishibosh located?

Answer: Cold Plains

Bishibosh is a unique Fallen Shaman but green instead of red. Fallen Shaman can revive Fallen, but Bishibosh can also revive other Fallen Shaman.

The Cold Plains is the first area outside of the Rogue Encampment with a Waypoint. It comes right after the Blood Moore, which is the first area beyond the Rogue Encampment. The incorrect answers were areas from other acts.

Answer: Kill the Countess

The quest structure in "Diablo II" is pretty straightforward. The first quest is to kill all the monsters in the Den of Evil, the second is to kill Blood Raven, the fourth is to kill the Countess, the fifth involves killing the Smith, and the final quest is to kill Andariel.

Answer: Horodric Malus

The Smith is another Super Unique monster that resides in the Monastery Barracks, and guards the Horodric Malus. The Horadrim were a major order in the lore of "Diablo", and left behind many artifacts. By the time the games take place, Deckard Cain is the only remaining member.

The three incorrect answers were from the "Warcraft" series, also by Blizzard.

Andariel is the only act boss in the game to have a negative resistance, and it is only to fire damage. Bringing fire based weapons and spells will help a lot in the fight against her. Andariel's primary damage type is poison.

Answer: Desert

The safe city in Act II is called Lut Golien which is located on the eastern edge of the Anorach Desert. The artwork for the act is heavily inspired by Middle Eastern and North African architecture and culture.

Answer: The Lost City

The Tainted Sun quest is given as you walk into The Lost City and the world darkens. It is not triggered by talking to an non-player character (NPC), unlike most quests. It is the result of Claw Vipers, so you seek out their temple and smash the Viper Alter in it to restore the sun. This also gives the Viper Amulet, the final piece of the staff for a different quest in Act II.

Answer: The Summoner

The Summoner is actually the Sorcerer from "Diablo". All three original heroes met unfortunate ends. The Rogue became Blood Raven, whom you killed in Act I. The Warrior became the Wanderer, who carried Diablo's Soul Stone within his forehead.

Answer: The Horadric Staff

The second quest in Act II is to assemble the Horadric Staff. First, you have to collect one of the most important items int the game: the Horadric Cube, which allows you to transmute (combine) items to create new/better items. Then you have to find the Staff of Kings and the Viper Amulet, and combine them in the Cube to create the Horadric Staff. Finally, you have to explore the correct Tomb of Tal Rasha, and place the staff in an inset to open the passage to the final boss of the act.
10. (Act II) What dose Duriel, the final boss of Act II, resemble?

Answer: A mutant giant maggot

Duriel is a large maggot-like creature, but with a vicious grin, and scythe-like arms. His attacks focus on cold damage, so be sure to bring thawing potions with you to the fight.
11. (Act III) Each act in "Diablo II" has a different kind of terrain. What kind of biosphere is featured in Act III?

Answer: Jungle

Act III takes place in the jungles of Kehjistan, which include the overgrown ruins of the city of Kurast. Unlike the open areas of the previous two acts, Act III's areas are quite narrow and twisting, often with rivers running down the middle. The Barbarian's Leap ability and the Sorceress' Teleport ability are quite useful for crossing the rivers.

Other classes have to go around or find bridges.

Answer: Gidbinn

Hralti gives this quest. The enchantment protecting the docks is weakening, and he needs the blade to revitalize it. In the Flayer Dungeon, where the Gidbinn is collected, you can also find Khalim's Brain, which is one of four items required for the quest Khalim's Will.
14. (Act III) Where do you fight the Act Boss for Act III?

Answer: Durance of Hate

The Lord of Hate resides in the Durance of Hate. It is an unpleasant dungeon, filled with torture devices, lots of spikes, and pools and moats of blood.
15. (Act III) The final boss of Act III is also the first of the Prime Evils you fight. What is his name?

Answer: Mephisto

Mephisto is the Lord of Hate. Like all Act Bosses, he is very challenging. His attacks focus on lightning damage. His brothers are Diablo, the Lord of Terror, and Baal, the Lord of Destruction.

Answer: Hell

You have chased Diablo across fields, deserts, marshes, and now into Hell itself. Act IV is a shorter act than the others, and is very much a rush to the endgame.

Answer: River of Flame

Hephasto guards the Hellforge and wields the Hellforge Hammer. You need to kill him, and then use the hammer to destroy Mephisto's Soulstone to finally defeat the Prime Evil.

All the other acts in "Diablo II" had six quests, but Act IV was the final area, and only had three. The "Lord of Destruction" expansion pack added Act V, which had the full six quests.

Anya was secreted away and frozen in ice by Nihlathak, and the Super Unique monster Frozenstein was left to guard her. Malah provides the potion to cure her, and Nihlathak slips away from Harrogath.

Nihlathak had secretly helped Baal to Mount Areat in exchange for sparring the city of Harrogath.
23. (Act V) For the fifth quest, Rite of Passage, you have to prove themselves worthy of entering the Worldstone Keep by defeating the Barbarian Ancients in combat. How many Barbarian Ancients do you have to fight?

Answer: 3

The three barbarian ancients are Madawc, Talic, and Korlic. Madawc uses throwing axes, Talic uses a sword and shield, and Korlic uses a giant two-handed axe. All three attack together, but they do not have any minions or spells you have to worry about.
24. (Act V) During the final quest, Eve of Destruction, Baal will summon five waves of enemies to fight you first - one for each act. Each wave is led by a commander. Which of the following Super Unique monsters is not a commander for one of these waves?

Answer: Illidan the Betrayer

The waves are led by Colenzo the Annihilator, Achmel the Cursed, Bartuc the Bloody, Ventar the Unholy, and Lister the Tormentor. After the final wave is cleared, players can face off against Baal, the last of the Prime Evils.

Illidan was a major character from the "Warcraft" series, also created by Blizzard.
