In Brooklyn in 2019, Anna attended a group for women seeking support going through IVF and for general life counselling. It's there that she met Siobhan and, commiserating over some of the melodramatic grief of one of their fellow group members, Siobhan offered that they head out together to grab a drink. Anna couldn't, however, since she had an audition in the morning. Nonetheless, she put her number into Siobhan's phone. Before leaving, Siobhan recommended Dr. Hill to her, insisting that he was the best fertility doctor in the city.

Offstage at the Oscars, Anna is met by Dex and Kamal who, having been called by Siobhan, join her as she's put on a stretcher and wheeled to the ambulance. Anna has to ask how Siobhan knew she'd won before they announced her name. There's no real answer for this; Siobhan says she'll meet her at the hospital.

Answer: Ivy

Anna, loaded into the ambulance, realized quickly that something was wrong. Commenting on a lack of paramedics seated with her and Dex in the back of the vehicle, she grips onto her Oscar and grits her teeth through the pain. In the front seat, Kamal asks the driver which hospital they're headed to, not recognizing the driver as Ivy. Ivy, taking the opportunity, grabs the gun from Kamal's holster and shoots him in the head, maintaining her control of the ambulance.

Anna, screaming that the baby was coming, tells Dex to do something-- anything-- to help her. When he positions himself to help, reaching in to try to retrieve the newborn, it's to both their shock and dismay that his hand is bitten off by the creature inside her. Ivy only watches from her rearview mirror and smiles.

Dr. Hill, at one time, was visited in his office by none other than Siobhan Corbyn, and he was given an offer he couldn't refuse, though initially, he was skeptical about taking it. Instead, she held a taloned nail to his throat, told him that she would send him all the high-profile clients in New York City, and give him everything he dreamed of. All he needed to do was whatever she said.

As Ivy drives Anna's ambulance through Los Angeles, she makes her way to a parking facility and into an empty warehouse, finally stopping the vehicle and clapping for Anna's efforts. Dex, in pain over the loss of his hand, writhes in the corner of the ambulance. They have, at the least, arrived at their destination.

Answer: His own hand

Rooting through Kamal's pockets, Ivy reveals his wallet, letting Anna know that her former security guard was a father; it's probably something she never knew out of her sheer selfishness. Ivy can only laugh at the situation. As Ivy heads around the vehicle and opens the back of the ambulance, she grabs Dex's severed hand, shoves it into his mouth, and chokes him to death on it. She says she feels better now, especially since Dex will never speak again.

As Anna screams in pain, the ambulance disappears from around her, and she looks up to find herself on the stretcher in the middle of the empty warehouse. Women garbed in black and led by Ivy surround her. They are the last minutes, she says, before Anna becomes a mother.

Answer: Her award

As the women in black continue to circle Anna, and chant, Ivy tells her to push and birth the baby she's been growing inside her for the past seven months. All it takes is the final push. Ivy grabs the child and cuts the umbilical cord, singing a lullaby while the others remove the placenta, leaving Anna on the gurney. Anna asks to be able to hold the baby, but Ivy says she's already holding what's hers; she chose the award, not the baby, even if she claims that she didn't mean to.

As the baby begins to cry, Ivy nurses the child herself. Anna rises from the stretcher, despite the strain, but is stopped as a long fingernail pierces her back. She falls to the ground, unaware of what may have stopped her, only to find, as the lights turn back on, that she has no idea what caused her to collapse. As the women sedate her, Ivy recommends she just stop fighting and surrender. It will be easier when she does.

Answer: Paralysis

Anna awakens sitting up in bed with Ivy, Sonia, Talia, and Nicolette surrounding her, all playing with Summer Day dolls. They refer to her as a 'sister', but the affectation doesn't help her come to terms, especially when she realizes that she can no longer move her legs, having had one of her vertebrae severed after she gave birth. According to Talia, she'll never walk again. A wheelchair is brought into the room for her.

As the women explain, they recruit women who want to have it all. Anna was perfect for them; they exploit women like her for something special. Hauling her to her wheelchair, they bring her down the hall to their leader, Siobhan, who's waiting to explain how everything was part of the plan.

Answer: Siobhan

Siobhan reasons with Anna that everything that has happened to her in the past years has been done for her and not to her. The paralysis, she admits, is a bummer, but it's because Anna reneged on their deal; she promised a baby and tried to take him back. Anna realizes that blood remains on her hands, literally and proverbially, but the women say that she can have them cleaned if she joins them. She has a choice like all others who serve their purpose, otherwise she can age and fall out of favour, not unlike Mavis Preecher.

Dexter's father enters the room with Anna's baby, revealing himself to be a long-term partner of Siobhan's. In fact, he was part of the plan as well. Decades earlier, she conceived of a child through him, none other than Anna's former husband. He was the perfect tool, but he was still a human. Together, they all managed to create the vessel that could create the most powerful creature of them all-- the baby Anna gave birth to. With Siobhan's egg and Dexter's DNA, they used Anna to create the perfect being without the need for more magic.

As the baby cries, Anna begs to hold it. Siobhan allows this, but only under the condition that she join the others.

Answer: Ivy

Siobhan gathers all of her Delicates in another room before the newborn and, while the others chant, Anna watches on. Ivy, taking her place before the baby, is struck in behind by Siobhan's taloned hand, a recompense for killing her son, even though Siobhan asked her to do so. It was a test, she claims. Ivy killed Dexter for Siobhan; she even killed her own daughter, Adeline, for her. None of it matters, however, when Siobhan slits her throat.

It's only after this that Siobhan reveals her master plan to Anna, unveiling the parts of her son that she kept intact, allowing her to continue to utilize the necessary body parts, along with her eggs, to use her Delicates (Anna included) as vessels for more perfect children. Her goal: to save the world, breeding creatures with special abilities to restore humanity to its greatest. It will be a matriarchal utopia.

Answer: Light

Anna can only ask who Siobhan really is, and Siobhan admits, she's the auteur of this world. It's hers to run. Siobhan says that it's time to hold the baby, after all, someone died for Anna. But as Anna grabs the child, she sees that it is not completely human.

Siobhan wheels Anna to a mirror in another room, insisting that she needs to see what she looks like the first time she nurses the child. The baby is terrible, however, drawing blood as it latches on for its feeding. Anna laments trusting Siobhan and falling for her tricks, but Siobhan says it doesn't matter. They're blood-bound and will never be apart again.

As Siobhan heads out to fetch libations for the two of them, Anna is visited by a woman she never met-- Adeline. As Adeline, dressed all in white, begins to chant, she encourages Anna to join her. She whispers into Anna's ear that only light can snuff out the dark.

When Siobhan returns to the room and finds Anna in Adeline's care, she begins to shake in place, feeling the effects of the pair's chanting immediately. Before Anna's very eyes, Siobhan disintegrates, aging into dust. The mirror cracks behind her and she rises from her wheelchair unaided, no longer paralyzed. As she looks down to the baby in her hands, she finds that she carries an empty swaddling blanket.

All that remains is Siobhan's feathered headpiece. Anna picks it up and dons it on her head.

Hearing crying in another room, Anna wanders the halls and finds her baby in a crib. A spider crawls on its bedding, and she takes it in her hand, drops it on the floor, and squashes it underfoot.

As Anna lifts her child in her arms, she looks to a nearby shelf and finds her Oscar. She looks down at both and wonders.
