Alice, having spent a bit more time in Joseph Peach's other world, realizes that things are very obviously different. She feels weird, but in a good way; her head finally has a bit of clarity. Joesph, sitting with Alice in the fields, tells her that all of it may be lost because the higher power that gave the Peach Family the summer house demands what it was promised-- the offering. It needs Izzy as a sacrifice, but she got away and they need her that very evening. Alice indicates that she understands. After leaving the Peach Family's world 'upstairs', Izzy was able to make it to her hiding spot. Zoe escapes as well, but she's followed soon after by the entire Peach Family.

While Zoe recaptured the loose bug, the sisters heard the sound of breaking glass coming from their mother in another room. Alice is broken from her memory by the sound of her phone ringing. It's not Zoe though-- it's a collections agency. She tells the person on the other line that Alice is dead.

Louise and Luke, having decapitated a number of Peach Family members, think about what to do next before there's a knock at the door. Luke loads his gun before they head up to the door and, on the other side, they find Izzy in Diane's care. Diane recommends that they keep the child safe when the Peach Family arrives, but she does not accept the invitation to enter the house. Zoe arrives soon after while Louise starts to clean the child up. She is, of course, back without Alice.

Something horrible draws her towards it, and as she enters the brightly-lit white room and nears the door, the horrifying thing within scares her away. Outside Garrett, Luke's father leads Peach's men out to the field where Robert and his son remain in the open grave. He explains that he believes in family and retribution, but when they check the pit, Luke's body is gone. The truth is, Louise retrieved Luke from the grave and brought him back to her house. With her skills in taxidermy, she sews his throat closed and saves his life.

As he pulls some of his crops from the soil next to him, Alice notices that he's not growing vegetables; it appears he's unearthing a cluster of human hands. At the summer house, Zoe sits for a meal with the Peach Family. Edie tries to lighten the mood, but Zoe claims that she isn't hungry. The rest of the family asks after Robert, but he hasn't been back from 'downstairs' since the day before.

Louise, curious about events in Rachel's recent days, asks her to go for a drive so that they can investigate exactly what the girls saw earlier. They retrace their steps, returning to Izzy's house, but it's locked. Rachel regrets letting Zoe head out. While Louise wonders if she went back to meet with their mother, Rachel says she wouldn't as she hadn't seen their mother since they locked her away. Rachel and Louise head into the park to find the spot where Joseph Peach's house once stood, right in front of a derelict playground. Louise says that the Peach Family disappeared in 1952. When workers came by and broke down the door, no one was inside. People blamed a cult; the Peach daughters were hanged in Butcher's Block; immigrant workers started disappearing. Others burned the house to the ground.

According to the story, the two graffiti artists split up. While Kara stayed behind to look for the figure hiding in the bushes, Ben went off on his own, coming across a fog-enshrouded playground lit by dim lampposts. There, he found the person hiding in the bushes waiting for him with a meat tenderizer in hand. Kara would later chase after him, hearing his screams, only to find no sign of him. After the chat, Alice heads back to her AirBNB. She and her sister, Zoe, have yet to find a new place being new to the city, and she's still in bed. When she wakes up, she crushes up some pills.

Alice heads into the house to look for Zoe and she finds that she appears to be under the covers in bed. Zoe talks about when their mother used to read 'Alice in Wonderland' to them. Unfortunately, she continues to dodge any questions about Peach. Alice finds evidence of drug use in the room and when she pulls back Zoe's bedsheets (despite Zoe's extreme hesitance) she finds that she's bleeding from her leg. It's not as bad as it looks, at least according to Zoe. Downstairs, Louise asks Nathan if he likes Alice and he seems to stutter out that he likes her professionally. He decides to deflect by asking about her taxidermy instead.

Alice and Zoe end up moving into an apartment owned by Louise Lispector, a woman who invites them in with a non-nonsense attitude, but the girls first meet another woman with more concerning words for them. The other woman, Diane, asks the girls if they have any scissors so that she can remove her bandages though the girls don't see her wearing any bandages. Diane also warns the girls not to leave the neighbourhood, at least not down past the park, because the people in Butcher's Block will cut them up like a pizza and eat every slice. Louise claims that Diane is 99% harmless.

Zoe ends up out in the fields to find the crops growing there. Horrified with the writing masses of flesh there, she looks up to find the gardener. She asks him about the red door and he complains that it's not a riddle. She phrases it differently. 'I'm not a peach. I fly. I get past the red door. What am I?' His answer: A child. The scratches in the walls lead Alice downstairs and into the room where Joseph Peach saved her from the manifestation in her mind, and when she gets there she finds the bloodstains still on the floor. Placing her hand on one of the walls, it seems to push back towards her until something tears through. It's Izzy. She asks Alice if she can go home, but Alice pushes her back inside; she has to wait just a little bit longer.

Alice continues to trudge onward through the fields and, inevitably, she comes across a man harvesting hands from the ground. He welcomes her and speaks of a 'secret companion' she seems to have brought with her, but she doesn't understand what he means. He gives her a riddle: 'I'm often shut but I'm not a mouth. Sometimes I break but I am not a heart. Sometimes I'm in pain but I do not feel it. When I am stained I am not unclean. What am I?' He gives her the answer immediately after: a window. Alice asks the man how to get closer to the house and he gives her another riddle: 'I am framed but not guilty. I have locks but no hair. I'm only closed when I'm open. And my jam will not spread on any toast.' She leaves without answering.

The police arrive at Medallion Park to hear Rachel's concerns about the staircase and the figures she saw earlier, but they don't take her seriously. In fact, they end up arguing with Louise about her invasive journalism and their ranks on the force. Elsewhere, a creature from the staircase crushes bones into a paste, spreading them on Izzy's face in the center of a demonic circle. When Rachel and Louise return home, they hear sounds deeper inside the building and, investigating, it turns out to be Zoe. Dressed in fine clothes and done up in make-up, she assures her sister that everything will be fine because she's been cured.

After a quick introduction to Louise's hairless cat, Andre, the girls are taken to their new room. Alice gets a call from her frantic mother who, a week too late, wishes her a happy birthday. Her mother asks where they moved, but Alice refuses to tell her. She begs to be taken out of the institution where they had to leave her, but Alice won't do it. The conversation ends there.

She doesn't seem to get any closer to the house though. Eventually, Alice comes across another person in the field, and it turns out to be Izzy, coated in something white. Izzy spies Alice's manifestation behind her and runs, but Alice doesn't see it yet, so what follow is a chase.

During the girls' first night in the new house, Louise works in the basement, performing taxidermy on a new project. Alice comes down to chat, asking that Louise not offer Zoe any drinks during their stay because she's in addiction recovery, and Louise agrees but insists that she not be bothered. The girls, she says, are just renters and she never had kids by choice. The next day, when Alice starts her new job, she's immediately driven into the Butcher's Block neighbourhood to be present for a CPS visit. On the drive through she asks about Peach's Meats and she's told that its owner, Joseph Peach, disappeared many years ago and the local meat packaging industry fell to pieces in his absence. It led Butcher's Block to become a 'sacrifice zone'-- a socioeconomic dead zone with little funding. Once again, she's encouraged to avoid going to Butcher's Block alone or at night.

After starting a fire in front of the stone towers, formerly the Peach Chapel, in Medallion Park, Edie begins to feel her first contractions. She's led through the park by the skinless man. Joseph and Alice, meanwhile, carry Izzy's body into the park to the fires; the paste is prepared by Joseph's wife. As Joseph and Alice rub the paste onto the girl, Alice asks if she'll see their God. Joseph says she will, but her mind is fragile, so she will want to close her eyes first. Zoe, Louise, and Luke make their way into the park to find Diane already watching and waiting. They agree that they will do whatever they have to. Soon, it becomes time. Alice and the Peach Family cover their eyes and await their God.

Louise tries to work with haste as Aldous quickly finds the locked basement door and tries to break through. Aldous, in doing this, doesn't notice the gun being pointed at his head from behind. It's Luke's dad though. He finds that Louise has fixed up his son and stolen Robert's head in the bowling bag and he tries to explain that he did what he did out of panic; the Peach Family isn't the mob-- they're worse. Luke and Louise explain that Aldous and the others will only get back up, even if he shot them, and they need to remove their heads. Zoe tells Alice that the Peach family is looking for a kid, but Alice lets her know that it's okay. They said that they plan to let her go. Soon, they're retrieved by the skinless man. It's time to go, Alice says.

Louise and Luke commit to finishing off Robert once and for all, and using power tools, Louise gruesomely remove Peach's head. When all is said and done, they sit down and discuss their next steps. Luke tries to convince Louise that they can trust his father, but she isn't quite sure. Luke is sure of his choice though, and he believes that his father will choose them over the Peach family. With Robert's head in a bowling ball bag, he departs. At the summer house, Zoe, having not indulged in lunch, is urged to eat her dessert-- kidney parfait and ear wafer. She can't though, and Edie takes hers to eat for herself. Can't let it go to waste.

At Louise's, Rachel and Zoe argue over Zoe's alleged curing; Rachel says that no one has just immediately cured schizophrenia before and claims that this is just a symptom of her illness. Rachel asks about Zoe's new clothes as well, and Zoe tells the truth. She met a man and he talked to her about something she can't quite remember. She was standing in a field and there were thousands of flies, and the flies came together and said that he could fix her. And he did. Rachel suggests they leave, but Zoe says she can't because she feels better. Besides, she says, if Rachel gets sick, she can be there for her instead. Later, alone, Zoe finds herself affected by the process that cured her. She remembers being told that the side effects would change her.

At her first field job, Alice has to deal with a mother whose child is going to be taken away by CPS. The woman claims that her daughter, Izzy, was bit by a dog in the park and though Nathan doesn't care whether or not this happened, Alice tries to calm down the mother with a story of her own. This does the trick. During this time, Alice seems to sense something odd. None of them know that there's someone inside the walls. Alice leaves the room and follows the noises through the hollow structure until she reaches Izzy's room. The girl seems to be wearing a red robe with the label 'Smart Mouth' inside. Alice asks about this and Izzy says she found it on the playground at the park.

Although Alice is told to keep her eyes closed, she doesn't. When she removes her hands, she sees that the staircase has appeared before them between the ruins of the chapel and this time, at the top, she sees the red door. She looks back at Izzy nervously while the door opens. Watching from the bushes, Zoe and Louise attempt to get closer and Zoe steps out to save her sister. Interrupting the ceremony, Zoe asks Alice how often the God needs to eat, but she does not know. While they talk, Diane steals Izzy off the altar without anyone noticing. She races off into the night, potentially spoiling the sacrifice. Joseph watches on nervously with his son's head in his hands. He tells Zoe that she just killed her sister as a horned being steps out onto the stairs.

Alice and Zoe are brought to meet with Joseph Peach and he is found standing at an altar with the rest of the family watching on. While Zoe sits nervously to the side, Alice is placed atop the marble altar and her head is dipped back into a cistern of blood. Joseph places a pair of pliers into her mouth and extracts the same thing he did for Zoe-- a centipede. The ceremony is declared a success though Zoe is still very worried. In Louise's basement, Luke's father saws into Aldous Peach to decapitate him. He stares back at his son while he does so.

Luke brings Robert's head home to his father, getting there to find his dad making a microwave dinner. When Luke opens the bowling bag, his father looks inside and finds Robert's head staring back at him. He asks where the rest of the body is, and when they go out to the cruiser they throw the bag in with the rest of the corpse. Luke has to ask his father-- what side is he on? 'I'm made of wood but I'm not a table. I have a handle but I'm not a brush. I do not live within a house nor do I live without. What am I?' This time when Alice comes across the farmer in the field, she's able to answer his riddle. It's 'a door'. When she realizes this, she turns around and finds three doors.

Racing through the hospital, Nathan is separated from the others by a locked, automatic door. Louise and Luke have no choice but to watch as Joseph Peach's son stalks down the hall behind him and slits his throat. Luke arrives in the hallway moments too late and handcuffs Peach while he laughs menacingly. Louise suggests that Luke bring Peach to her place for questioning. In her room, Zoe watches as the power shuts off and when she's left in the dark she screams for help. Unfortunately, there's no one around to help. She disconnects herself from her monitors and plans to leave moments before a woman appears before her bearing food.

In jail, the man known as Peach cracks his fingers as the other man in the cell with him asks him to stop. This escalates and Peach makes it clear that this will become a fight. When Luke returns to the holding cells, he finds that Peach has overpowered the man in the cell, removed his own clothes, and pierced into the man's side to feed on the organs within him. As a clean-up crew comes in, Luke sees the image of a mouse drawn in the blood on the floor. This Peach had nicknamed him 'Mouse' shortly before.

Zoe, now frustrated, walks through the summer house and down the hallways ready to face the red door and when she arrives she does not let herself be frightened by the presence within. Instead, she walks right up to the portal and opens it, forcing her way inside. She emerges into blackness and finds a single light within where Izzy lays in the center of an eight-pointed star on the floor. She lifts the girl up and races away with her while a demonic presence with antlers watches from a distance. Zoe races back out the red door and through the summer house, but the place is a labyrinth. They end up at a doorway and, sure enough, it's a staircase leading downstairs to their world-- the real world. Zoe sends Izzy through, telling her to meet at Louise's house if she doesn't return.

Alice, having left her sister behind, finds a storage room of the hospital and helps herself to some of the medication inside. She remembers a discussion she had with a therapist before she and her sister moved to Garrett where she worried that, like her mother and sister, she could be at risk of schizophrenia more than the 9% value on the published data. As she lifts herself from the floor, she turns around to find the manifestation of schizophrenia that Joseph Peach pulled from her mind. And what follows is a terrifying chase through the abandoned hospital.

To his surprise, instead of being sent to another cell, the man is removed from his handcuffs outside Medallion Park and set free. Luke decides to follow him out to the playground, nearly getting caught in the process, and he watches as Peach is received by his father and they ascend the stairs. Back at Louise's, Zoe has a horrifying vision. Unable to quench the urges within her, she struggles with the image of eating Louise's pet cat. No one in the house hears as someone enters in the night
