In the years after Sebastian's escape from Beacon, he not only loses his daughter in a horrible house fire, but splits up with his wife, Myra, and descends into a spiral of self-deprecation, binge-drinking, and depression. The game picks up when Castellanos is given an opportunity, from double agent Juli Kidman, to help MOBIUS, a shady agency, re-enter STEM and locate his daughter's consciousness. As it turns out, she never died in the fire, and he might be the only one who can save her and their project. He accepts the offer, of course.

After Sebastian reenters the world of the STEM, he's left off in the professional office of his mind and then asked to make his way to Union, the constructed town within this version of the STEM where his daughter is supposed to be. While Kidman communicates to him via a comm device, he is expected to locate Mobius forces, missing in Union, and connect with them to accomplish the goal. This starts with O'Neal, whose safe house is the perfect place from which Sebastian can begin locating clues and unlock the truth behind what happened in the town.

The mission isn't so simple. As Sebastian navigates through the seemingly all-American town of Union on his hunt for Lily, he discovers that the place is swarming from creatures not unlike what he faced in Beacon. Additionally, a handful of MOBIUS operatives have gotten scattered throughout the area. The first he finds is O'Neal, who has a safehouse in Union. The goal, to begin, is for Sebastian to use his finely-tuned communicator to locate and track resonances found throughout Union, following Lily's movements from when she was in the area.

Green Gel comes in a handful of forms throughout the game. While you're likely to find Green Gel in tidy, glass jars, the substance is also dropped by enemies upon being killed or being burned and destroyed. Green Gel is used when Castellanos goes into the alternative hospital realm. In addition to saving in this relatively safe space he can, after the first chapter, gain access to a room with an upgrade chair. Each upgrade costs Green Gel and can affect weapons, ammunition, health, strength, or carrying capacity depending.

Heading through the gate with the doctor from the hospital, Chapter 4 brings Det. Castellanos to an abandoned hospice to search for Leslie, who was once treated there years ago. An insane Dr. Valerio will be waiting inside, but the chapter will progress into the basement of a village house where the Detective will be separated from Leslie and the good doctor by the hooded figure and be sent back to the decrepit halls of Beacon Mental Hospital. The chapter ends with a frantic chase with Laura, an optional boss with long, black hair who crawls on many legs. She can be killed with enough power, but she's not altogether necessary to kill. As long as the detective can survive long enough to separate himself from her, he can continue the descent into madness.

To access Tatiana's office, Sebastian needs to find the hallway in his own office that contains a chair, and when he sits in it he'll be transported into her realm complete with a wall of lockers and options for upgrading different aspects of play from stamina and health to weapon abilities and stealth tactics. Like in "Evil Within", Green Gel is used to upgrade all facets, but jars of Red Gel can also be found throughout the gam to unlock further tiers of upgrades, much like the high quality weapon parts for the bench.

This chapter sends Sebastian back into the Marrow in order to navigate his way to what appears to be the next destination of focus-- the Grand Theatre. It requires a bit of a deep dive though; Sebastian will have only encountered the Marrow once or twice before, barely scratching the surface of the labyrinthine corridors now full of foes. At the end of the path, he'll come across another safe house, this one held by MOBIUS psychologist Yukiko Hoffman, whose terminal connects to a Post Plus location near the Grand Theatre.

Inner Recesses takes place throughout the wards of what must be Beacon Mental Hospital and it's certainly a more concerning place than ever before since regular enemies are accompanied by larger foes, some who wield explosives, and some who are just outright invisible. These final class of foes will usually make themselves known through movement and slight shifts in the atmosphere and can be spotted out with Flash Bolts from the Agony Crossbow. This chapter also features a final boss battle against Laura, as previously seen in Chapter 4, and reunites Det. Castellanos with Joseph and Kidman, both of whom are separated from him again by the chapter's end.

Arriving in this section of Union, Sebastian needs to not only (likely) secure a safe house in the middle of the area, but dodge guardians and all sorts of enemies throughout the city streets. It will all come down to following the resonances to two locations: the Sanctuary Hotel and the Devil's Own Taproom. Both of these spots are on opposite sides of the map, but each one contains a painting that needs to be destroyed before access to the next destination is granted.

The Agony Crossbow, first able to be found in Chapter Three, is a versatile weapon that has the ability not only to fire unique ammunition, but to have its ammunition made on the fly from the inventory screen using parts salvaged from disabled traps. In addition to a basic harpoon shot, the Agony Crossbow also fires Explosives, Flash shots and Freezing and Shocking bolts. Each of these acts in separate pools for ammunition purposes, but you need to swap between them to use them. The "Fighting Chance" DLC pack (and game, if pre-ordered) also adds Poison bolts and Incendiary shots for the weapon. While the Crossbow itself can be upgraded, so too can each individual ammo type.

Det. Castellanos reunites with Joseph early in this chapter as the nightmare takes them both through the old ruins of a village, somehow modernized with electronic amenities, in pursuit of Kidman, who's being taken through to the other side. What results is a trip through a maze-like village interspersed with hordes of foes and ended with a pair of giant aberrations-- twins-- who use their bulk to fight. The detectives will drop into a laboratory in the end and both will have to face off against a mutated, giant dog before reaching the end of the chapter, but not before both of them collect sniper rifles, all the better to take out enemies at a distance and with precision.

After fighting off foes in the chapel, the story will drive Det. Castellanos down a ladder to an underground river and cave system, and the journey through these caves (and oddly-placed chambers of stone) will be a short one. That said, this is also the first instance in which he'll come across smaller enemies-- ones that can be stepped on to be squished. The end of the chapter will teleport Sebastian back into the hospital for a short time...and then things get really dire.

Ultimately, Sebastian will finally hone in on his daughter, locating her in a part of the STEM that's separated from Union, but through the journey he'll come to realize that she's in the care of her mother-- his former wife, Myra-- who entered the STEM to save her. The STEM will have corrupted her by this point, however, and the final boss battle against 'The Matriarch' will result in her maternal instincts manifesting as a horrible creature. If Sebastian defeats her he can escape with Lily while Myra sacrifices herself and destroys Mobius. Despite this, however, it seems like something still stirs within the STEM.

Sebastian is sent to the ruinous remains of his family home in pursuit of lily and Father Theodore and the experience is overwhelming enough that he is unable to see the difference between reality and delusion. When Sebastian finally catches up to his daughter, he fires at Theodore but ends up hitting Torres in his confusion back in the Marrow. He ends up retreating into his memories while Torres gets him back to Hoffman's safe house and succumbs to her wounds, and in doing so he receives a supportive and motivating message from Myra asking that he forgive himself for what's happened to Lily.

To be honest, Hoffman is the only MOBIUS operative still remaining by this point as all of the others will have died during the course of the story or, in Sykes' case, taken a chance (in a side mission) to return himself to the real world. In the meantime, Father Theodore creates a fiery stronghold near the Grand Theatre, and to get inside, Sebastian and Hoffman need to make use of O'Neal's final invention, a mobile Field Stabilizer, to break through. Sebastian will make it, but a late rush of enemies will keep Hoffman down and she won't reach the stronghold in time to avoid being consumed by flames.

Getting through the sanctuary to the top, Sebastian will vow to save his daughter and incite the wrath of Father Theodore, whose goal is to control Union and MOBIUS with it to, in the end, control the world. It will lead to a sequence in which Sebastian is sent back to Beacon to face off against the foe with the chainsaw, The Keeper, and the multi-legged boss from "The Evil Within". All of them will be forms taken on by Father Theodore and, in the end, his plan will go up in flames. Myra will arrive to deal the final blow against him before destroying the Sanctuary (and, soon after, Union) and leaving Sebastian in the rubble.

After the crash, there's nothing much to do but, once again, reunite with the others, and Det. Castellanos has to continue through the buildings and streets as they close in around him, crumbling away and exposing new traps and powerful foes. Amongst these traps are obstacles that contain large tanks of burning, green acid which, like all traps before, can be sabotaged and salvaged for parts. Electricity traps show up another chapter in the future. In the end of this chapter, Kidman holds Leslie hostage in a small parkette. She won't succeed in killing him and ending the nightmare, but everyone will split up once again as the streets break down around them.

Sebastian will rush to find Lily, but there's more going on in the real world as the MOBIUS operatives come to find that their simulation has failed. Kidman, still on Myra and Sebastian's side, works to take down MOBIUS from within as the collapse of Union occurs. She runs through the compound and locks herself into the STEM chamber to begin the extraction. Sebastian and Lily flee while Myra, gravely injured, stays back to put an end to MOBIUS, and as they escape through the mirror in Sebastian's office, all of MOBIUS' men are eliminated via the chips in their heads. Sebastian and Lily head out and Kidman goes her separate way while back in the compound, some time later, STEM turns itself back on.
