1. Chapter 1: An Emergency Call Detective Castellanos, after being attacked in the hospital, goes head-to-head against a person wielding which of these?

Detective Castellanos and his colleagues are sent to Beacon Mental Hospital to investigate a strange occurrence and find that many have been slaughtered inside. During a quick search of the premises, Castellanos is attacked and imprisoned in the basement and forced to make his way through the sewers and decrepit tunnels beneath the structure to escape, but this does involve some harrowing attempts to escape a person known only as 'The Butcher', who wields a chainsaw and pursues him through the bowels of the hospital. If he can escape up the elevator and return to the main lobby, he can get out of the hospital only to face an earth-shattering quake that threatens to kill him and his remaining colleagues.

The escape ends when their ambulance plunges off a cliff.
2. Chapter 2: Remnants This chapter marks the first appearance of which of these?

Answer: Green gel

After the introductory first chapter of the game, Det. Castellanos picks up almost immediately after the ambulance crash and heads through the woods in pursuit of Leslie, a patient from Beacon Mental Hospital. The trip takes him through some shacks and coastal residences, but for the most part, the way is blocked by a number of undead nightmares, and this chapter is designed to figure out the basic mechanics of the game (including weapons, stealth kills, and the healing).

This is also the first chapter in which the Detective can awaken in the Safe Haven to save his progress or upgrade himself using green gel.
3. Chapter 3: Claws of the Horde During this chapter, if you access the save area, you can enter the morgue and use which of the following?

While much of Chapter 3 involves a tough fight with the chainsaw-wielding 'Butcher' of Chapter 1, there is the possibility of Det. Castellanos starting off with a trip through the looking glass to Safe Haven where he can upgrade himself and his weapons, save the game, and visit the morgue, a room behind the counter where he can spend any accumulated keys to open morgue drawers. Each one contains a random item ranging from ammunition to syringes to green gels. Keys can be found, hidden, throughout the game.

The rest of this chapter involves navigating a small village, avoiding traps, and killing a fearsome foe locked in the stables (at least temporarily) to escape a locked gate.
4. Chapter 4: The Patient Another visit to the hospital will result in the first encounter with what optional foe?

Answer: Laura

Heading through the gate with the doctor from the hospital, Chapter 4 brings Det. Castellanos to an abandoned hospice to search for Leslie, who was once treated there years ago. An insane Dr. Valerio will be waiting inside, but the chapter will progress into the basement of a village house where the Detective will be separated from Leslie and the good doctor by the hooded figure and be sent back to the decrepit halls of Beacon Mental Hospital.

The chapter ends with a frantic chase with Laura, an optional boss with long, black hair who crawls on many legs.

She can be killed with enough power, but she's not altogether necessary to kill. As long as the detective can survive long enough to separate himself from her, he can continue the descent into madness.
5. Chapter 5: Inner Recesses In this chapter, some otherwise common foes will take on which interesting trait?

Answer: Invisibility

Inner Recesses takes place throughout the wards of what must be Beacon Mental Hospital and it's certainly a more concerning place than ever before since regular enemies are accompanied by larger foes, some who wield explosives, and some who are just outright invisible.

These final class of foes will usually make themselves known through movement and slight shifts in the atmosphere and can be spotted out with Flash Bolts from the Agony Crossbow. This chapter also features a final boss battle against Laura, as previously seen in Chapter 4, and reunites Det. Castellanos with Joseph and Kidman, both of whom are separated from him again by the chapter's end.
6. Chapter 6: Losing Grip on Ourselves What weapon is introduced in this chapter?

Answer: Sniper Rifle

Det. Castellanos reunites with Joseph early in this chapter as the nightmare takes them both through the old ruins of a village, somehow modernized with electronic amenities, in pursuit of Kidman, who's being taken through to the other side. What results is a trip through a maze-like village interspersed with hordes of foes and ended with a pair of giant aberrations-- twins-- who use their bulk to fight.

The detectives will drop into a laboratory in the end and both will have to face off against a mutated, giant dog before reaching the end of the chapter, but not before both of them collect sniper rifles, all the better to take out enemies at a distance and with precision.
7. Chapter 7: The Keeper Which of these must be found to make your way through the chapel and its catacombs?

Answer: Lithographs

The seventh chapter finally allows the detective to reach the chapel where Kidman has been taken and suffice to say, it's just as plagued with horrors as everywhere else is. In this chapter, he needs to descend into the catacombs collecting Lithographs to unlock additional chambers, many of which are rigged with traps and creatures.

When all is said and done, the Keeper himself, a creature with a safe on his head, will continuously attack through a series of rooms as toxic gas flows in. Shutting off the valves and killing Boxhead will allow Castellanos to escape.
8. Chapter 8: A Planted Seed Will Grow This chapter takes place almost entirely in which location?

Answer: Cave

After fighting off foes in the chapel, the story will drive Det. Castellanos down a ladder to an underground river and cave system, and the journey through these caves (and oddly-placed chambers of stone) will be a short one. That said, this is also the first instance in which he'll come across smaller enemies-- ones that can be stepped on to be squished.

The end of the chapter will teleport Sebastian back into the hospital for a short time...and then things get really dire.
9. Chapter 9: The Cruelest Intentions Which of these sections of the brain do you not need to locate throughout the house?

Answer: Pain

Although Chapter Nine takes the detective through the rooms and corridors of an old mansion, the goal is fairly straightforward-- he needs to unlock a door in the main lobby, and to do that he needs to find three hidden chambers containing brain cross-sections, and by probing into the consent, fear, and hope sections of the brains as per their medical diagrams, the goal can be accomplished.

The problem is that Ruvik is beyond the door, and he'll send the detective on a mind-bending trip through the beyond.
10. Chapter 10: The Craftsman's Tools There's a boss fight at the end of this chapter that takes place in which of these spots?

Answer: Parking garage

After spending most of this stage navigating through what seem to be abandoned warehouses, basements, and trapped utility rooms, there will be a fight against the creature known as Laura (which is aided by a lot of fire jets around the stage) before, ultimately, one larger boss battle against the creature unleashed in the end of Chapter Eight.

This fight, which takes place in a parking garage, simply requires firepower. It comes after a sequence in which Leslie's doctor tries to free the three of them from Ruvik's nightmare, but fails to do so due to interference.

He's killed by the beast in short time.
11. Chapter 11: Reunion Heading through the destroyed city, the detective will come across enemies that do which of these?

Answer: Swim

After a number of chapters that weave through old buildings and hospital corridors, the detective will leave the parking garage, dropping in an elevator, to emerge in the city streets, eventually coming across both Kidman and Joseph. This chapter not only features a clone of Ruvik in the busy streets, but several stretches of flooded roadways that contain hungry, swimming beasts. Distracting those creatures with handy hanging bodies is a necessity to make it through.
12. Chapter 12: The Ride This chapter takes place, for the most part, in which of these vehicles?

Answer: City bus

The bulk of this chapter involves navigating the city streets with Kidman and Joseph, the former driving the unwieldy vehicle and the latter helping you fight off a giant spider-like creature that pursues you down highways and between skyscraper-lined streets. Joseph will, at one point, require medical aid and you'll need to venture out to get to an ambulance (and that's not the safest idea), but for the most part, this stage is as linear as it gets until Ruvik gets in the way and brings things to a screeching halt.
13. Chapter 13: Casualties Traps of which type are prominently featured for the first time in this chapter?

Answer: Acid

After the crash, there's nothing much to do but, once again, reunite with the others, and Det. Castellanos has to continue through the buildings and streets as they close in around him, crumbling away and exposing new traps and powerful foes. Amongst these traps are obstacles that contain large tanks of burning, green acid which, like all traps before, can be sabotaged and salvaged for parts. Electricity traps show up another chapter in the future.

In the end of this chapter, Kidman holds Leslie hostage in a small parkette.

She won't succeed in killing him and ending the nightmare, but everyone will split up once again as the streets break down around them.
14. Chapter 14: Ulterior Motives The only way to get back to Beacon Medical Hospital is to cross a bridge made of which of these?

In the penultimate chapter, Detective Castellanos will continue diving into the depths of the city, maneuvering through dungeons and sewers containing dense webs, waterlogged passages, and electrical traps. All of this will lead to the collection of three fuses and, after a battle against a camouflaged, squid-like foe, an elevator trip to the top of the building he needs.

The chapter culminates with a trip across the long-seen-from-a-distance, but never reached bridge of subway cars that lead to Beacon Mental Hospital, and after leaping through them and minding the gap, he'll finally be where he needs to be.
15. Chapter 15: An Evil Within The final battle of the game involves a face off against Ruvik himself.

Answer: True

The final chapter begins on the final path to Beacon Mental Hospital and while there are few enemies along the path, much of the time is spent chasing Leslie through abandoned corridors until reaching a house on the property, deep within a courtyard. It leads to Ruvik, of course, and he sends Detective Castellanos on a trip through his own warped mind, forcing a battle royale against well-armed foes. When Sebastian passes through he eventually makes it to the Beacon proper and ascends to the top, leading to a final face-off against Ruvik in a terrifying final form that would kill the detective, regardless of outcome, if he didn't wake up from being hooked up to the STEM machine after destroying Ruvik's brain.
Detective Castellanos emerges from Beacon, still after the massacre that was held there, and when he does he sees Leslie leave through the front gates. No one else notices.
And still...it may not be over.
