1. Starting with the moves e4 e5 Nf3 Nc6, what is the opening called when White plays 3.d4?

Answer: Scotch

The Scotch opens up the center after exd4 Nxd4. Although it moves the same piece twice, it gives White a strong pawn in the center while Black's pawn in the center has already been exchanged.
2. The Caro-Kann defense (played against e4) is which of these moves?

Answer: 1...c6

Black's idea in the Caro-Kann Defense is to play d4 on the second move to challenge White's pawn on e5. The opening is somewhat passive and defensive, but it's a lot safer than some other openings.

Answer: Rossolimo

In the Nimzo-Indian (d4 Nf6 c4 e6 Nc3 Bb4) Black's idea is to give up the bishop pair and play against White's doubled c-pawns, while in the Rossolimo (e4 c5 Nf3 Nc6 Bb5) White's idea is to give up the bishop pair and play against Black's doubled c-pawns.

Answer: Sicilian

In the Sicilian (e4 c5), White can play the Alapin (c3) which plans to take control of the centre with d4.

Answer: c5

The Benoni is commonly reached after the moves d4 Nf6 c4 c5 d5 e6. So Black's c5 move could transpose into this opening.
The reason I said "has the best chance" is because technically the other moves could transpose into the Benoni, if White and Black both make a bunch of silly moves to get the position they want.
7. What move by Black initiates the Rubinstein Variation of the French defense?

Answer: dxe4

The Rubinstein variation is "e4 e6 d4 d5 Nc3 (or Nd2) dxe4 Nxe4. Or instead of Nxe4, White could sacrifice a pawn with f3, transposing into a variation of the Blackmar-Diemer Gambit.
8. Which of these is NOT an opening beginning 1. e4 d5?

Answer: Norwegian

The Scandivanian opening or Center-Counter defence are general names for 1. e4 d5.
The Blackmar-Diemer gambit is a main variant of the Scandinavian. White declines to take on d5, but rather offers his own pawn with 2. d4

There are at least two variants dubbed the Norwegian defence, against the Ruy Lopez opening and the Modern defence respectively. Neither starts with 1. e4 d5, however.
9. In the King's Indian Attack, what does White usually play on his first four moves?

Answer: Nf3 g3 Bg2 O-O

The King's Indian Attack is an opening for players who prefer a safer, more solid and flexible setup without any early conflict in the centre. White usually follows up the opening with d3 Nbd2 e4, or White could even play e4 on the first move and then d3 Nd2 before developing the kingside.
10. Which of these opening names is used to indicate, 1.d4 Nf6, 2.Nf3 e6, 3.Bf4?

Answer: London

In the London System, White wants the setup d4-e3-Nf3, and his plan is to get the queen's bishop developed before playing e3 which would block it in. The Colle is similar to the London, except White plays e3 without moving the queen's bishop, and can fianchetto it on b3 later.
