Although Lucy didn't die right there and then, she did die later because of complications with her surgery (a pulmonary embolism). Lucy was stabbed by a schizophrenia patient having a delusion. It's always Homer's fault. At the Springfield Raceway, Homer painted a target onto his stomach so the cheerleaders would shoot free T-shirts at him. When the cheerleaders did shoot one at him, it missed Homer and hit Maude pushing her off the top of the bleachers where she fell and died.
