In "Worst Case Scenario", a holodeck version of Janeway dies because of an overloaded weapon, overloaded by a holographic Seska.
In "Before and After", in an alternate timeline that Kes experiences, Janeway dies during the year of hell, the time period when the Krenim attack.
In "Deadlock", the real Captain Janeway dies to escape the Vidiians while the duplicate Captain Janeway (along with most of the duplicate crew) survive.

2. Who do we know graduated from Starfleet Academy?

Answer: Commander Chakotay

Janeway mentions this in the season 1 episode "Parallax". The other Maquis either dropped out, never attended, or we don't their status.

3. Who is a mix between two species?

Answer: Torres

Torres has a Klingon mother and a human father. The Doctor has no parents, although one might consider Dr. Zimmerman to be his "father", since he was the one who created the Emergency Medical Holograms. Tuvok had Vulcan parents and Harry Kim had human parents.
